AMENDMENT TO CITY SECRETARY
CONTRACT NO. 33726
WHEREAS, on June 14, 2006, the City of Fort Worth ("City") and Fort Worth Housing
Authority ("Contractor") made and entered into City Secretary Contract No. 33726 (the
"contract");
WHEREAS, the contract provided funding to Contractor pursuant to a grant received by
City from the United States Department of Housing and Urban Development ("HUD") through
the Community Development Block Grant ("CDBG") Program No. B-06-MC-48-0010, Catalog
of Federal Domestic Assistance ("CFDA") No. 14.218, to provide services to the citizens of the
City;
WHEREAS, one of the national objectives of the CDBG program is to benefit low and
moderate income citizens (the"National Objective");
WHEREAS, the citizens of Fort Worth, the Community Development Council, and the
City Council of Fort Worth have determined that CDBG programs are needed by low and
moderate income citizens of Fort Worth;
WHEREAS, it the mutual desire of City and Contractor to extend City Secretary Contract
No 33726 for an additional four months to September 30, 2007.
NOW, THEREFORE, City, whose address is 1000 Throckmorton St., Fort Worth TX
76102, acting by and through Dale A. Fisseler, its duly authorized Assistant City Manager, and
Fort Worth Housing Authority, whose address is P O Box 430, Fort Worth, TX 76101
("Contractor"), acting by and through Barbara Holston, its duly authorized Executive Director, do
hereby agree as follows:
I.
1. Scope of Services This section is amended to be and read as follows:
"a. Contractor will administer services to citizens of the City of Fort Worth under the
program name AMAKA Child Care ("Program"), for the term June 1, 2006 to September 30,
2007."
All other terms and conditions of City Secretary Contract No. 33726 not amended herein
remain unaffected and in full force and effect, are binding upon the parties, and are hereby
ratified by the parties.
