AMENDMENT TO CITY SECRETAItY
CONTRACT 1v0. 33723
WHEREAS, on June l, 2006 the City of Fort Worth ("City") and YWCA of Fort Worth
and Tarrant County. ("Contractor") rnade and entered into City Secretary Contract No. 33723
(the "contract");
WHEREAS, the contract provided funding to Contractor pursuant to a grant received by City
from the United States Department of Housing and Urban Development ("HUD") under Subtitle
B of Title IV of the McKinney-Vento Homeless Assistance Act, as amended, (42 USC 11371 et
seq. ) in connection with the Emergency Shelter Grants ("ESG") program, Program No. 5-06-
MC-48-0006, for Year X��I;
WHEREAS, one of the national objectives of the ESG program is to benefit low and
moderate income citizens (the "National Objective");
WIIEREAS, the Contract expires by its terms on May 31, 2007;
WHEREAS, it the mutual desire of City and Contractor to amend Ciry Secretary Contract
No. 33723
NOW, THEREFORE, City, whose address is 1000 Throckmorton St., Fort Worth TX
76102, acting by and through Dale A. Fisseler, its duly authorized Assistant City Manager, and
YWCA of Fort Worth and Tarrant County (Supportive Living program), whose address is 512
West 4`'' Street, Fort Worth, TX 76102 ("Contractor"), acting by and through Judi Bishop, its
duly authorized Executive Director, do hereby agree as follows:

4. Re�ortin� Procedures this section is amended to be and read as follows by adding new
subsection f.
f. Contractor shall participate in a local Continuum of Care's Homeless Management Information
System ("HMIS") to ensure that the Program meets the Federal goal that localities collect an
array of unduplicated data on the number and characteristics of homeless persons. Continuums of
Care are local bodies that plan for and coordinate homeless services. HUD's standards for
participation in a local HMIS are set forth in the Federal Register, July 30, 2004. The local
Continuum of Care is Fort Worth/Arlington/Tarrant County CoC (TX-601).
