(No M&C Needed) COUNTY OF TARRANT S WHEREAS, the City of Fort Worth (City) and TCB, Inc., (Engineer) made and entered into City Secretary Contract No. 34097, (the Contract) which was authorized by the City Council by M&C C-21714 on the 26th day of September 2006; and WHEREAS, the Contract involves engineering services for the following project: Engineering Design Services: Street Reconstruction, Water an.d Sewer Replacement on Edney Street (Hemphill St to S. Adams St and 6�' Ave to Sharondale St) and Southcrest Drive (Townsend Dr to W. Dead End), 2004 CIP Contract Year 4& 5, Contract 54, Project No. 00448; DOE No. 5408 WHEREAS, it to said Contract maximum fee; has become necessary to execute Amendment No. 1 to include an increased scope of work and revised NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1. Article I of the Contract .is amended to include the additional engineering services specified in a proposal letter dated December 20,2006, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $8,030.00. Below is a summary of services to be provided under this request: 1. Take seven (7) borings to a depth of ten (10) feet; 2. Backfill borings with bentonite chips / sacrete; 3. Perform laboratory testing on samples including Atterberg limits, moisture content, continuous PP strength / swell profile, swell testing, sulfates and lime series; 4. Prepare geotechnical report with recommended pavement and subgrade sections in accordance with the current City of Fort Worth Pavement Design Standards Manual. TCB proposes to utilize Mas-Tek Engineering & Associates, Inc. for the geotechnical work and will perform these services for a lump sum fee of $8,030 as shown in the breakdown below: Mas-Tek - $7,300 10% Markup - 730 Total - $8,030 Should you have any questions or need additional information, please contact our Project Manager, Bryan Jann, at 817.698.6787.
