TO CITY SECRETARY CONTRACT `��,�(�� ����
WHEREAS, the City of Fort Worth (CITY) and Teague Nall and Perkins,
Inc., (ENGINEER) made and entered into City Secretary Contract No. 34801,
(the CONTRACT) which was authorized by M&C C-21918 on the 9th day of
January, 2007 in the amount of $668,038.00; and
WHEREP,S, the CONTRACT was subsequently revised by: Amendment Number
1 in the amount of $19,000.00 which was administratively authorized by
the City on the 19th day of June, 2007; and subsequently revised by
Amendment #2 in the amount of $89,000.00 which was authorized by the City
Council on the 18th day of March, 2008 (M&C C-22712); and subsequently
revised by Amendment #3 in the amount of $190,400.00 which was authorized
by the City Council on the 3rd day of February, 2009 (M&C C-23325); and
subsequently revised by Amendment #4 in the amount of $50,000.00 which
was administratively authorized by the City on the 20th day of December
2010; and subsequently revised by Amendment ##5 in the amount of
$113,128.00 which was authorized by the City Council on the 6th day of
December, 2011 (M&C C-25310); and
WHEREP,S, the CONTRACT involves engineering services for the
following project:
Keller-Hicks Road from 750 feet west of Ridgeview Street to Park
Vista Boulevard and North Beach Street from Golden Triangle to
Keller-Hicks Road, Project Nos. 00653, 00791 and 01455; and
WHEREAS, it has become necessary to execute Amendment No. 6 to the
CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT:

Article I of the CONTRACT is amended to include the additional
engineering services specified in a proposal dated November 18, 2012, a
copy of which is attached hereto and incorporated herein. The cost to
City for the additional design services to be performed by Engineer
totals $132,115.00.

Article II of the CONTRACT is amended to provide for an increase in
the fee to be paid to Engineer for all work and services performed under
the Contract, as amended, so that the total fee paid by the City for all
work and services shall be an amount of $1,261,681.00.
3.
All other provisions of the Contract, which are not expressly
amended herein, shall remain in full force and effect.

Fee Proposal for Contract Amendment No. 6(Revised)
Dear Mr. Weiss:
Teague Nall and Perlcins, Inc. is submitting this tee Amendment to the above referenced contract
based on additional services requested. The following is a brief description pnd cost for the
additional services provided on this project:

(Golden Triangle Boulevard fo Keller Hicks Road)
As requested, engineering services was provided up to and during the Condemnation hearing
involved with the Drainage Easement and Construction Easement on Ihe Strand Property. Engineering
Services involved a pre-coordination meeting, preparation of exhibits for the condemnation hearing,
briefings with the City Attorney, and testifyiny pt the candemnation hearing. Also requested was the
re-staking of the Drainage Easement, preparation of a floodplain exhibit for the Hillwood tract, pnd
the preparation of the right-of-way acquisition exhibits for the triangle parcel near the proposed
intersection of Altp Vista and North Beach Street. After Final plans were issued for North Beach Street
and the project was under construction, the City of Fort Worth Water Department requested TNP to
perform evaluations and design for interconnection of major trpnsmission mains near the intersection
of North Beach and Keller Hicks. Numerous options and designs were pursued, including transferring
a neighborhood in the Villpges of Woodland Springs to a higher pressure plan, which required
additional survey along Park Vista. The originpl contract and previous nmendments anticipnted 30
ROW parcels. This amendment accounts for the preparation of 1 additional ROW pprcel pnd 1
water easement. Construction staking along Keller Hicks (Phase 2) for the road and utility
improvements will be provided. The scope for construction staking includes staking the paving offsets
twice and the utility (water, sanitary sewer, and drainage) offsets once during the course of
construction. Additional construction staking or restaking will be considered additional services.

RECOMMENDATION:
It is recommended that the City Council authorize the execution of Amendment No. 6 to City Secretary
Contract No. 34801, an Engineering Services Agreement with Teague Nall and Perkins, Inc., in the
amount of $132,115.00 for the design of Keller-Hicks Road from seven hundred fifty feet west of
Ridgeview Street to Park Vista Boulevard and North Beach Street from Golden Triangle Boulevard to
Keller-Hicks Road, thereby, establishing a total contract in the amount of $1,261,681.00.
DISCUSSION:
This contract Amendment No. 6 is necessary for the design of newly added water improvements,
streetlights, associated right-of-way acquisition, and additional construction management services. This
amendment will also provide for revision of the construction specifications and details to meet the
recently updated City standards.
Funds for this amendment will be provided from 2007 Certificates of Obligation, 2008 Capital
Improvements Program, and Water Capital Projects.

Teague Nall and Perkins, Inc., proposes to perform these professional services for a lump sum fee of
$132,115.00. Staff considers this fee to be fair and reasonable for the scope of services to be
performed.
MWBE Office: Teague Nall and Perkins, Inc., is in compliance with the City's M/WBE Ordinance by
committing to an additional eight percent MM/BE participation on this Amendment No. 6. The City's
goal on this amendment is seven percent.
This project is located in COUNCIL DISTRICT 2, MAPSCO 22J and 22N.
FISCAL INFORMATION/CERTIFICATION:
The Financial Management Services Director certifies that funds are available in the current capital
budget, as appropriated, of the Street Improvements 2008 Fund, the 2007 Critical Capital Projects
Fund, and the Water Capital Projects Fund.
