WHEREAS, the City of Fort Worth (CITY) and Teague Nall and Perkins,
Inc., (ENGINEER) made and entered into City Secretary Contract No. 34801,
(the CONTRACT) which was authorized by the City Council by M&C C-21918 on
the 9�' day of January, 2007; and subsequently revised by Amendment #1
which was authorized by the City on the 19�' day of June, 2007; and
subsequently revised by Amendment #2 which was authorized by the City
Council by M&C C-22712 on the 18�' day of March, 2008;
WHEREAS, the Contract involves engineering services for the following
project:
Keller-Hicks Road from 750 feet west of Ridgeview Street to Park
Vista Boulevard and North Beach Street from Golden Triangle
Boulevard to Keller-Hicks Road;
WHEREAS, it has become necessary to execute Amendment No. 3 to said
Contract to include an increased scope of work and revised maximum fee;
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the Contract:

Article I of the Contract is amended to include the additional
engineering services specified in the proposal letter dated October 16,
2008, copies of which are attached hereto and incorporated herein. The
cost to City for the additional design services to be performed by
Engineer total $190,400.00.

Fee Proposal for Contract Amendment No. 3
Dear Mr. Weiss:
Teague Nall and Perkins, Inc. is submitting this fee Amendment to the above referenced contract
based on additional services requested. The following is a brief description and cost for the
additional services provided on this project:
Additional Services to provide surveying and design for the east half of UNIT 2: Keller
Hicks Road (Alta Vista 1p Park Vista Boulevard),
North Beach Street (Golden Triangle Boulevard to Keller Hicks Road)
As requested, the right of way along the east half of North Beach (Phase 3) and a portion of Alta
Vista will need to be surveyed between Golden Triangle and Keller Micks to adequately design the
eastern half of North Beach Street. Also, the original contract anticipated 19 ROW parcels and
there were actually 22 parcels along Keller Hicks. This amendment accounts for the 3 additional
ROW parcels along Keller Hicks Road, 10 construction easement exhibits along Keller Hicks Road
and 3 ROW parcels along the east side of North Beach, near the merger with Alta Vista. The
additional design services consist of designing the eastern half of North Beach Street paving and
the associated drainage improvements. Design of any water or sanitary sewer main extensions or
relocations is excluded. This amendment also provides a wetland delineation and evaluation for
compliance with NWP —14 LinearTransportation Projects, with no notification to the USACE. This
project is likely to be authorized by this NWP. However, if the project exceeds the criteria for
notification underthis NWP, the required notification to and coordination with the USACE and/oran
individual permit is specifically excluded from thisscope of services. In addition, this amendment
includes the preparation and coordination of the USACE Pre-Construction Notification document
package required for the wetlands disturbance at the new culvert under Keller Hicks Road (Phase 2)
as mentioned on page 6 of the Wat�rs of the United States Delineation Report prepared by
Integrated Environmental Solutions, Inc., dated July 31, 2008.

RECOMMENDATION:
It is recommended that the City Council authorize the City Manager to execute Amendment No. 3 in
the amount of $190,400.00 to engineering services agreement City Secretary No. 34801 with Teague
Nall and Perkins, Inc., for the design of Keller Hicks Road from 750 feet west of Ridgeview Street to
Park Vista Boulevard and North Beach Street from Golden Triangle to Keller Hicks Road thereby
increasing the total amount to $966,438.00.
DISCUSSION:
2007 Certificates of Obligation provided funds for Teague Nall and Perkins, Inc.'s current design
contract for the west half (two lanes) of North Beach Street, and the 2008 CIP provides funds for the
east half (two lanes) of North Beach Street. Therefore, Amendment No. 3 is required for Teague Nall
and Perkins, Inc., to complete the design of North Beach Street as a four lane divided arterial street
from Keller Hicks Road to Golden Triangle Boulevard.
On March 18, 2008, (M&C C-22712; City Secretary Contract No. 36890) the City Council authorized
the City Manager to execute Amendment No. 2 in the amount of $89,000.00 for hydraulic analysis,
wetlands determination, conditional letter of map revision (CLOMR), and letter of map revision (LOMR)
for Big Bear Creek at Keller Hicks Road and waterline improvements from Park Vista Boulevard to
2,000 feet west of Katy Road.
On June 19, 2007, Amendment No. 1(City Secretary Contract No. 35400) was administratively
approved in the amount of $19,000.00 for additional surveying for right-of-way and construction staking
for Keller Hicks Road from 750 feet west of Ridgeview Circle to Alta Vista Road.
On January 9, 2007, (M&C C-21918, City Secretary Contract No. 34801) the City Council authorized
the City Manager to execute an engineering services agreement with Teague Nall and Perkins, Inc., in
the amount of $668,038.00 for the design of Keller Hicks Road from 750 feet west of Ridgeview Street
to Park Vista Boulevard and North Beach Street from Golden Triangle to Keller Hicks Road.
The professional services included in Amendment No. 3 are summarized below:
Survey, preparation of right-of-way and easement documents, design of the eastern half of North
Beach Street, and wetlands delineation at Big Bear Creek in the amount of $163,000.00 (2008 CIP
funds); and
Preparation and coordination of United States Army Corps of Engineers' Preconstruction
Notification for the proposed culvert at Big Bear Creek and the preparation of right-of-way and
easement documents for Keller Hicks Road from Alta Vista to Park Vista in the amount of
$27,400.00 (2007 C.O. funds).
Teague Nall and Perkins, Inc., proposes to perform these professional services for a lump sum fee of

$190,400.00. City Staff considers this fee to be fair and reasonable for the scope of services to be
performed.
Teague Nall and Perkins, Inc., is in compliance with the City's M/WBE Ordinance by committing to an
additional 16 percent M/WBE participation on this Amendment No. 3. The City's goal on this
amendment is 16 percent.
This project is located in COUNCIL DISTRICT 2, Mapsco 22J through N.
FISCAL INFORMATION/CERTIFICATION:
The Financial Management Services Director certifies that funds are available in the current capital
budgets, as appropriated, of the 2008 Street Improvements Fund and the 2007 Critical Capital
Projects Fund.
