SECOND EXTENSION TO CITY SECRETARY
CONTRACT NO 31678
THIS SECOND EXTENSION to City Seci•etary Contract No. 31678 (hereinafter `Bxtension") is
entered into by and between the City of Fort Worth ("City"), a home-iule municipality in the State of
Texas, acting through Libby Watson, its duly authorized Assistant City Manager, and Industrial
Hygiene and Safety Technology, Inc., a Teaas Department of State Health Services Licensed
Asbestos Consultant Agency, acting tluough Tracy K. Bramlett, its duly authorized President
("Contractor").
WHEREAS, the City and Contractor entered into that certain agreeinent identified as City Secretary
Contract No. 31678 (also the "Contract") commencing on April 1, 2005 and ending March 31, 2006
with two successive one-year extensions for the provision of Professional Services for Asbestos,
Mold, and Industrial Hygiene Consulting Services; and
WHEREAS, the City and Contractor wish to exercise the second extension option;
WHEREAS, the parties agree that all other conditions, terms and pricing shall reinain tlie same as
stated in City Secretaiy Contract No. 31678.
NOW, THEREFORE, for and in consideration of the muhial covenants contained herein, and other
good and valuable consideration, the receipt and sufficiency of which are hereby acicnowledged and
confessed, the parties hereto agree as follows:
The Agreement lcnown as City Secretary Contract No 31678 shall be extended pursuant to Section 4
"Term of Contract; Time to Start and To Complete Taslc Orders" for its second one (1) year
extension beginning April 1, 2007 through March 31, 2008.
