FIRST AMENDMENT TO
FORT WORTH CITY SECRETARY CONTRACT NO.50184
This First Amendment to Fort Worth City Secretary Contract No. 50184 ("First
Amendment") is made between the City of Fort Worth ("FORT WORTH"), a municipal
corporation, acting herein by and through Susan Alanis, its duly authorized Assistant City
Manager and mySidewak Inc., a Delaware Corporation ("mySidewalk") acting herein through
Stephen Hardy,its duly authorized CEO.
WHEREAS, FORT WORTH and mySidewalk entered into an Agreement identified as
Fort Worth City Secretary Contract No. 50184 beginning January 24, 2018 (the "Agreement");
and
WHEREAS,mySidewalk wishes to extend the subscription agreement through December
26,2019 at a cost of 12,000.00; and
WHEREAS, it is the collective desire of both FORT WORTH and mySidewalk wishes to
amend the Agreement to include annual cost increases for each renewal exercised.
NOW THEREFORE, KNOWN ALL BY THESE PRESENT, FORT WORTH and
USER, acting herein by the through their duly authorized representatives, enter into this First
Amendment, which amends the Agreement as follows:
1.
The Agreement is hereby amended by adding Exhibit A, attached to this First
Amendment, as Exhibit A of the Agreement for the mySidewalk subscription renewal through
December 26,2019.
2.
The Agreement Exhibit B is amended to include current year and renewal year
subscription costs as Exhibit B-1,attached to this First Amendment.
3.
All other terms, provisions, conditions, covenants and recitals of the Agreement not
expressly amended herein shall remain in full force and effect.
