SUBJECT: REQUEST TO FILE DOCUMENTATION TO CITY SECRETARY
CONTRACT NO. 36689
BACKGROUND
On February 28, 2019, the City Secretary's Office received Amendment No. 3 to City Secretary
Contract No. 36689 from Municipal Courts Department. After research done by the Administrative
Assistant it was discovered that Amendment No. 1 was not on file. It was asked that the department
provide Amendment No. 1. After researching from Administrative Service Manager, Juan Paredes, it
was discovered that this Amendment was not in their records anymore and they could not locate a
copy of this amendment. Correspondence related to the location of the original and the request for
filing has been included in the contract file.
AUTHORIZATION FOR CONTRACT NUMBER
Per the direction of Mary Kayser, City Secretary, the City Secretary's Office has been requested to
issue a contract number, and place in the City of Fort Worth's official contract files Amendment No.
1 that was missing.
This request has been approved.

We had talked about the Sentinel Amendment No. 1 and you had stated you would reach out to Purchasing and see if they
had Amendment No. 1 so that way the contract file for 36689 could be complete.I have not heard back from you and I
still have amendment No. 3 here in my office waiting for some sort of response so that way we can proceed with this
amendment.Please let me know what you found out. Thank you.

I received Amendment No. 3 with Sentinel but when I went to the file I only see Amendment No.2.Do you by chance
