FIRST AMENDMENT TO
FORT WORTH CITY SECRETARY CONTRACT NO. 51974
This First Amendment is made between the City of Fort Worth ("FORT WORTH"), a
municipal corporation, acting herein by and through Jesus J. Chapa, its duly authorized Assistant
City Manager and Texas Historical and Ancestry Researchers and Gayle W. Hanson (the
"Contractor"), an Individual, located at P.O. Box 122058, Arlington, TX 76012. The City has
designated the Arts Council of Fort Worth & Tarrant County, Inc., to manage this Agreement on
its behalf.
WHEREAS, on February 28, 2019, the City and Contractor entered into City Secretary
Contract ("CSC") No. 51974 whereby Contractor agreed to provide professional research and
writing services (the"Agreement");
WHEREAS, Contractor's business name is not currently registered as a business entity
with the Internal Revenue Service; and
WHEREAS, City and Contractor desire to amend the Contract to include the correct
references and descriptions to the Contractor;
NOW THEREFORE, in consideration of the mutual covenants herein expressed, the
parties agree to amend the Contract as follows:
I.
The parties wish to revise all references to the Contractor replacing "Texas Historical and
Ancestry Researchers" with "Gayle W. Hanson," where applicable. The Agreement shall be binding on
Gayle W. Hanson as if originally executed by Gayle W. Hanson, and Gayle W. Hanson shall be entitled
to all payments under the Agreement as outlined in the Agreement.
II.
All terms and conditions of the Agreement that are not expressly amended pursuant to this First
Amendment shall remain in full force and effect, and any undefined terms in this First Amendment shall
have the meaning provided in the Agreement
IN WITNESS WHEREOF,the parties hereto have executed four copies of this First Amendment
in Fort Worth, Texas,to be effective this !7h day of—1\n►^j ,2019("First Amendment
