SUBJECT: REQUEST TO FILE CHANGE ORDER MISSING TO CITY SECRETARY
CONTRACT NO. 50166
BACKGROUND
On May 14,2019,the City Secretary's Office received Change Order No. 3 to City Secretary Contract
No. 50166. During routine research before filing, it was discovered that Change Order No. 1 was
never filed with the City Secretary's Office. Per M&C C-29081 presented before Council on April
2, 2019, Change Order No. 1 was proposed but never executed. Therefore there is no Change Order
No. 1 paperwork to be filed. Change Order No. 2 is on file with our office as Change Order No. 1 but
should not be numbered that way. Therefore, it is requested that this memo be made the original in
order to facilitate the processing of subsequent Change Orders. Correspondence related to the
location of the original and the request for filing has been included in the contract file.
AUTHORIZATION FOR CONTRACT NUMBER
Per the direction of Mary Kayser, City Secretary, the City Secretary's Office has been requested to
issue a contract number, and place in the City of Fort Worth's official contract files the Change Order
No. 1.
This request has been approved.

Project: Cattle Barn 1 and the Poultry Barn Renovations Project
Remarks: The purpose of Change Order No. 3 is to finalize the contract amount. There were
$267,283.25 in allowances that were not expended. The original contract amount was
$14,248,486.00. Change Order No. 1 was proposed but never executed.
Change Order No. 2, in the amount of$516,475.00, was authorized by City Council, on
September 25, 2018, (M&C C-28861), and increased the total contract amount to
$14,767,961.00. Change Order No. 3, in the amount of-$267,283.25, reduces the final
contract amount to $14,497,677.75. A 1.02% increase to the original contract amount.
