Final Payment Application (Payment No. 12)
Dear Ms. Walters:
Please find attached one copy of the Final Payment Application (Payment No. 12) for work performed
by Lindamood Demolition, Inc. on the City of Fort Worth Riverside Wastewater Treatment Plant
Demolition and Sludge Lagoons Excavation Project through April 30, 2019. The attached invoice includes
all charges for the project to reflect 100%final completion of all contract items and payment for the 5%
retainage held by Lindamood Demolition during the contract. In addition, Lindamood Demolition has
submitted the following additional contract documentation for the project:
• Contractor's Affidavit of Bills Paid
• Contractor's Affidavit of Payment of Debts and Claims
• Contractor's Affidavit of Release of Liens
• Consent of Surety to Final Payment
We have reviewed the application (Final Payment Application No. 12) and recommend that payment be
made in the amount of$382,494.58 (which includes $50.175.00 in charges to complete the project and
$334,828.33 in gross retainage).

except for those noted on the attached schedule. Any unpaid future obligations will be
paid and satisfied directly out of funds paid in reliance on this affidavit; and
The wage scale established by the City Council in the City of Fort Worth has been
