Contract No. CSC No. 50414 (the "Contract")
Renewal Term No. 1: March 20, 2019 to March 19, 2020
The above referenced Contract with the City of Fort Worth expires on March 19,2019 (the
"Expiration Date"). Pursuant to section 4.2 of the Contract, the Contract "shall renew
automatically at the expiry of the Initial Term or the then-current Renewal Term for an additional
one (1) year periods." Section 4.3 of the Contract states that if the Contract"is not renewed before
the end of the Term, it will terminate at the end of the Term."
To avoid any ambiguity,this letter is to inform you that the City is requesting to renew the Contract
for an additional one (1) year period, which will begin immediately after the expiry of the Initial
Term and end on March 19, 2020. All other terms and conditions of the Contract remain
unchanged.
To agree to renew the Contract for an additional one (1)year period, please sign and
return this contract renewal notice, along with a copy of your current insurance certificate,
to the address set forth below.

that your company information is correct and up-to-date.
