TO CITY SECRETARY CONTRACT
WHEREAS, the City of Fort Worth (CITY) and
(ENGINEER) made and entered into City Secretary
CONTRACT) which was authorized by M&C C-22181
2007 in the amount of $ 43,216.00; and

WHEREAS, the CONTRACT was subsequently revised by: Amendment Number
1 in the amount of $11,060.00 which was administratively authorized
authorized on November 24th, 2008, Amendment Number 2 in the amount of
$39,385.00 which was administratively authorized on October 25th, 2010;
and
WHEREAS, the CONTRACT involves engineering services for the
following project:
Quail Run Drainage Improvements, Project No. 00629; and
WHEREAS, it has become necessary to execute Amendment No. 3 to the
CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT:
1.
Article I of the CONTRACT is amended to include the additional
engineering services specified in a proposal dated December 14th, 2011, a
copy of which is attached hereto and incorporated herein. The cost to
City for the additional design services to be performed by Engineer
totals $5,352,00.
2.
Article II of the CONTRACT is amended to provide for an increase in
the fee to be paid to Engineer for all work and services performed under
the Contract, as amended, so that the total fee paid by the City for all
work and services shall be an amount of $99,013,00,
3.
All other provisions of the Contract, which are
amended herein, shall remain in full force and effect.

Dear Mr. Simmons:
Teague Nall and Perkins, Inc. is submitting thisAmendment to the above referenced contract based
on additional services requested. The following is a brief description and cost for the additional
services provided on this project:
Additional Services for Inlet Relocation
Based on comments received at the pre -construction neighborhood meeting and direction
froIII the City we will relocate the inlet at 5324 Quail Run St. such that when it is re-
constructed it will be in the same general location as the existing inlet. This change will
require revisions to the following plan sheets:
1 —Project Layout, General Notes, Control Data, &Legend;
3 —Erosion Control Plan;
8 — Hydraulic Tables;
9 — SD Line 'A' Plan and Profile;
10 — SD Line 'B' & LAT 'B-1' Plan & Profile;
11 — SD Line 'C' Plan & Profile (BEGIN to STA 3+50);
12 — SD Line 'C' Plan and Profile (STA 3+50 to END);
13 — Storm Drain Laterals; and,
14 — WL 'A' & 'B' Plan & Profile;
The ENGINEER shall provide an initial plan and profile sheet for construction of the inlet
and provide the balance of the changes with Record Drawings at the completion of the
project.

Additional Services for Wedge Mill and Overlay
Based on comments received at the pre -construction neighborhood meeting and direction
from the City we will provide a wedge mill and overlay detail for Quail Run St. We will also
provide quantities for the contractor to provide pricing for this additional work. Adding

additional pavement thickness to the street could impact gutter capacity and will require a
check of capacity and possible revisions to the hydraulic calculations and inlet lengths. The
ENGINEER will evaluate pavement thickness changes and make necessary adjustments to
the project plans.
