Shop drawing research, gate valve placement, and initial traffic control planning was started on this task but not completed per City's request. The deliverables for this task will be the survey work that was performed, in addition to the shop drawing research. The design and construction of this 36"cut-in gate valve will be performed and included in a future City project. • Additional design for 210 LF of 16" ring connection at the McPherson StNV.Clebume Rd intersection. This task includes multiple layout options to configure multiple fittings for horizontal and vertical alignments within the intersection and many various existing underground utilities (existing water line connection points, existing storm drain, existing gravity sewer, existing force main sewer, fiber optic, gas, &overhead lines). This task will also involve the preparation of an additional plan and profile sheet for this 16" ring connection. • Revision to construction sequencing drawings and preparation of the multiple additional construction sequencing steps and associated drawings as a result of the 16" ring connection in order to reduce the shut down time of the single Bethesda Water Supply Company , City of Burleson and City of Crowley water line feed. Additionally, as a result of the proposed ring connection and the subsequent down time to the City's customers, additional plan research was performed to determine the best location to provide a temporary supplemental water line feed from the Summer Creek Lift Station to the existing 36" water line on the east side of W. Cleburne Rd during the connection of the 42"tee to the existing 24"and 36"water lines. • Additional traffic control plan preparation and survey associated with 16" ring connection. This task will include surveying of approximately 600 LF west of the McPherson/W. Cleburne intersection, including the Toni Trail/McPherson Blvd Intersection for the preparation of the traffic control plan along McPherson Street. This task will also involve multiple additional phases of traffic control along W. Cleburne to accommodate the construction sequencing and the construction of the valve vault structure. This task will also involve providing temporary drainage as a result of the impacts to the adjacent bar ditch and the 24" SD line crossing McPherson Blvd when the additional temporary pavement widening is constructed. This task will also include additional coordination meetings with the Traffic Department. • Additional individual meetings with the City of Fort Worth Staff, Bethesda Water Supply Company, City of Crowley, & City of Burleson for construction coordination when transmission main is shut down for the proposed water line connection. This will also include an additional joint meeting prior to construction. Comments received will be incorporated into the construction sequencing plans. • Amendment No. 1 October 24, 2008 Page 3 A summary of the proposed budget for this contract amendment is as follows: Basic Fee 660 LF of 12"Water Line Design $ 6,600.00 M/WBE Survey $ 1,500.00 Sub Consultant Coordination 10% 150.00 200 LF of 8"Water Line Design $ 2,000.00 Detailed Traffic Control and Si na a Plans $ 14 971.00 M/WBE Survey $ 3,400.00 Sub Consultant Coordination 10% $ 340.00 Detailed construction sequencing Ian 4 004.00 36""cut-in"gate valve design $ 1,076.00 M/WBE Survey $ 11600.00 Sub Consultant Coordination 10% $ 160.00 210 LF 16"Ring Connection $ 6 971.00 Revised construction sequencing lan for Ring Connection $ 3,267.00 Detailed Traffic Control and Si na a Plans for Ring Connection $ 9,978.00 M/WBE Survey $ 2,035.00 Survev Credit from previous TCP survey at Hulen/St intersection $ (1,050.00) M/WBE Survey $ 985.00 Sub Consultant Coordination 10% $ 99.00 Customer Coordination Meetings& Follow-up $ 3,436.00 Alignment Report - Additional Re roduction $ 3,800.00 TOTAL PROPOSED AMENDMENT $ 64 337.00 A summary of the total project is as follows: Contract Description Fee Original Contract 42"Water Line Design $208,357.00 Amendment No. 1 Additional Water Line Design.Traffic control& $64,337.00 construction sequencing design&coordination.
