COUNTY OF TARRANT $
WHEREAS, the City of Fort Worth (City) and Stream Water Group, Inc. ,
(Engineer) made and entered into City Secretary Contract No. 34090, (the
Contract) which was authorized by the City Council by M&C C-21721 on the 26th
day of September 2006; and
WHEREAS, the Contract involves engineering services for the following
project:
Engineering Design Services: Street Reconstruction, Water and Sewer Replacement
on NE 37th Street (Grover Ave to Deen Rd) , Decatur Avenue (Terminal Rd to
Altamont Dr and Altamont Dr to Elaine P1) , Hardy Street (Elaine P1 to Mineola
Ave) and Selene Street (Decatur Ave to Weber St) , 2004 CIP Contract Year 4 & 5,
Contract 41, Project No. 00441; DOE No. 5396
WHEREAS, it has become necessary to execute Amendment No. 3 to said
Contract to include an increased scope of work and revised maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through their duly
authorized representatives, enter into the following agreement which amends the
Contract:
1.
Article I of the Contract is amended to include the additional engineering
services specified in a proposal letter dated September 18, 2007, a copy of
which is attached hereto and incorporated herein. The cost to City for the
additional design services to be performed by Engineer total $7,105.00.
2.
Article II of the Contract is amended to provide for an increase in the
maximum fee to be paid to Engineer for all work and services performed upde _ the
Contract, as amended, so that the total fee paid by the City .for all work- and
services shall not exceed the sum of $222,384. 61.
3.
All other provisions of the Contract which are not expressly amended
herein shall remain in full force and effect.

The following is a summary of additional work requested for Amendment No. 3 to the above referenced project,per
design meetings with City of Fort Worth officials.
The proposed amendment includes Model update for Decatur Rd Box Culvert location,additional survey,and
preparation of easement document project limit as shown in Exhibit 1.
Easement Document Preparation(9 ea)(by Subconsultant) $4,000.00
Additional Survey along Creek S 600.00
Model update and coordination for Decatur Rd Box Culvert S2,705.00
10%A WBE $ 400.00
Total Additional Services S7,705.00
Amendment No.3 TOTAL LUMP SUM $7,705.00
The current status of the contract between Stream Water Group,Inc and City of Fort Worth for the Street
Reconstruction,Water and Sewer Replacement,2004 CIP Contract Year 4&5,Contract 41 is as follows:
Original Contract Amount= $ 164,947.00
Amendment No. 1 Amount= $ 14,201.00
Amendment No.2 Amount= $ 35,531.61
Amendment No.3 Amount= $ 7,105.00
Total Contract Amount including Amendments= $222,384.61
Please process Amendment No. 3,to the Street Reconstruction,Water and Sewer Replacement,2004 CIP Contract
Year 4&5,Contract 41 in the amount of$7,705.00.
