AMENDMENT NO. 2

COUNTY OF TARRANT S
WHEREAS, the City of Fort Worth (City) and Stream Water Group,
(Engineer) made and entered into City Secretary Contract No. 32125 ,
(the Contract) which was authorized by the City Council by M&C C-20814
on the 28th day of June 2005; and
WHEREAS, the Contract involves engineering services for the
following project:
Engineering Design Services: Street Reconstruction, Water and Sewer
Replacement on Bertrose Ave (Mattison Ave to Dorothy In) , Clarke Ave
(Camp Bowie Blvd. To Virginia P1) , and Modlin Ave (Haskell St to West
Dead End) , 2004 CIP Contract Year 2, Contract 23, Project No. 00167: DOE
No. 4912
Water Project No. P253-0671700167
Sewer Project No. P258-0771700167
WHEREAS, it has become necessary to execute Amendment No. 2 to
said Contract to include an increased scope of work and revised maximum
fee;
NOW THEREFORE, City and Engineer, acting herein by and through
their duly authorized representatives, enter into the following
agreement which amends the Contract:
1.
Article I, of the Contract is amended to include the additional
engineering services specified in a proposal letter dated December 12,
2006, copies of which are both attached hereto and incorporated herein.
The cost to City for the additional services to be performed by Engineer
total $3,392.00.
2.
Article II, of the Contract is amended to provide for an increase
in the maximum fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall not
exceed the sum of $135,458 .00 .

3.
All other provisions of the Contract which are not expressly amended
herein shall remain in full force and effect.

Dear Mr. Bergeron:
The following is a summary of additional work requested for Amendment No. 2 to the above
referenced project, per recent design meetings with City of Fort Worth officials.
The proposed amendment includes additional survey, field check, and modification design
documents.
The total cost to perform the additional services included in Contract Amendment No. 2 is as
follows:
Sewer
Topographic Surveys S 597.00
Coordination Meeting $ 575.00
Field verification and Design Modification S 2,220.00

The current status of the contract between Stream Water Group, Inc and City of Fort Worth for the
Street Reconstruction, Water and Sewer Replacement, 2004 CIP Contract Year 2, Contract 23 is as
follows:
Original Contract Amount= $ 101,476.00
Amendment No. 1 Amount= $ 30,590.00
Amendment No. 2 Amount= $ 3,392.00
Total Contract Amount including Amendment No. 1 = $ 135,458.00
Please process Amendment No. 2,to the Street Reconstruction, Water and Sewer Replacement,
2004 CIP Contract Year 2, Contract 23 in the amount of$3,392.00.
