CITY SECRETARY CONTRACT NO. 37121
EVALUATION AGREEMENT FOR DATABASE CONSULTANTS OF AUSTRALIA INTERNATIONAL
SOFTWARE AND HARDWARE
WHEREAS, the City of Fort Worth (hereafter "City" or "Customer") and Database Consultants of
Australia International, LLC (hereafter "DCAI"), individually referred to as a "party," collectively referred to
as the "parties," entered into City of Fort Worth City Secretary Contract ("CSC") No. 37121, ("Contract")
on or about May 28, 2008; and
WHEREAS, the Contract involves the implementation, evaluation and ultimate removal of the
Product ("Pin Force" and "Sentinel") that contains software and hardware segments therein; and
WHEREAS, the parties previously entered into CSC No. 37499 amending the Contract to modify
the term and the compensation provision; and
WHEREAS, the parties no wish to further amend the Contract to extend the term.
NOW, THEREFORE, City and DCAI, acting herein by and through their duty authorized
representatives, enter into this Agreement which amends the Contract as follows:
1.
Schedule 1, Section 6, Evaluation Period is hereby amended to read as follows:
Evaluation Period: August 11, 2008 to December 24, 2008, but not to exceed 132 days from the
Commencement date.
2.
All other provisions of the Contract that are not expressly amended herein shall remain
unchanged and in full force and effect.
