COUNTY OF TARRANT §
WHEREAS, the City of Fort Worth (City) and Stream Water Group, Inc.,
(Engineer) made and entered into City Secretary Contract No. 34090 (the Contract)
which was authorized by the City Council by M&C C-21721 on the 26th day of
September, 2006. Amendment No. 1 for $14,201.00 was approved by administrative on
February 9, 2007 (City Secretary Contract No. 34885), Amendment No.2 for 35,531.61
was approved by City Council on September 4, 2007 (M&C C-22358, City Secretary
Contract No. 35798), and Amendment No. 3 for $7,105.00 was approved by
administrative on October 24, 2007 (City Secretary Contract No. 36009).
WHEREAS, the Contract involves engineering services for the following project:
Engineering Design Services: Street Reconstruction, Water and Sewer
Replacement on portions of NE 37' Street, Decatur Avenue, Hardy Street and Selene
Street (2004 CIP, Project No. 00441; DOE No. 5396);
WHEREAS, it has become necessary to execute Amendment No. 4 to said
Contract to include an increased scope of work and revised maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through their duly
authorized representatives, enter into the following agreement, which amends the
Contract:
1.
Article I of the Contract is amended to include the additional engineering services
specified in a proposal letter dated April 4, 20083 a copy of which is attached hereto and
incorporated herein. The cost to City for the additional services to be performed by
Engineer total $9,870.00
2.
Article II of the Contract is amended to provide for an increase in the maximum
fee to be paid to Engineer for all work and services performed under the Contract, as
amended, so that the total fee paid by the City for all work and services shall not exceed
the sum of $231,654.61
3.

Dear Mr. Sahu:
The following is a summary of additional work requested for Amendment No. 4 to the above referenced project, per
design meetings with City of Fort Worth officials.
The proposed amendment includes Subsurface Utility Engineering, (SUE), additional survey, and design
modification due to change in drainage construction plan and easements requirements.
Subsurface Utility Engineering (by M/WBE Subconsrrltant) $ 4, 600.00
Additional Survey along Decatur Ave Culvert $ 600.00
Plan sheets update (Drainage design Change) $ 4,210.00
10% M/WBE $ 460.00
Total Additional Services $9,87& 00

The current status of the contract between Stream Water Group, Inc and City of Fort Worth for the Street
Reconstruction, Water and Sewer Replacement, 2004 CIP Contract Year 4 & 5, Contract 41 is as follows:
Original Contract Amount = $164,947.00
Amendment No. lAmount = $ 14,201.00
Amendment No. 2 Amount = $ 35,531.61
Amendment No. 3 Amount = $ 79105.00
Amendment No. 4 Amount = $ 94870900
Total Contract Amount including Amendments = $ 2319654961
Please process Amendment No. 4, to the Street Reconstruction, Water and Sewer Replacement, 2004 CIP Contract
Year 4 & 5, Contract 41 in the amount of $9,870.00.
