GtG�s FIRST AMENDMENT TO
FORT WORTH CITY SECRETARY CONTRACT NO. 52197
This First Amendment to Fort Worth City Secretary Contract No. 52197 is made between
the City of Fort Worth ("Fort Worth"), a municipal corporation and AboveTraining, Inc. d/b/a
StateFoodSafety, a Utah corporation("Consultant").
WHEREAS, Fort Worth and Consultant entered into an Agreement identified as Fort
Worth City Secretary Contract No. 52197 beginning April 19, 2019(the"Agreement"); and
WHEREAS, it is the collective desire of both Fort Worth and Vendor to amend the
Agreement to articulate the product fee share between City and Consultant for additional
products.
NOW THEREFORE, known by all these present, Fort Worth and Vendor, acting herein
by the through their duly authorized representatives, agree to the following terms, which amend
the Agreement as follows:
1. The Agreement is hereby amended by adding Exhibit A, attached to this First
Amendment, as an additional page to Exhibit A of the Agreement.
2. All other terms, provisions, conditions, covenants and recitals of the Agreement
not expressly amended herein shall remain in full force and effect.

The pricing and fee share details defined herein apply to purchases made
through the City's SFS-provided web portal, fortworth.statefoodsafety.com.
SFS may employ temporary price changes in conjunction with its promotional
strategy for certain times, locations, products,or services at its discretion. The
pricing agreed herein shall not be subject to change under these or any other
circumstances unless explicitly agreed in writing signed by both the City and
SFS.

d. Reporting
i. Products Purchased and Certificates Awarded.
i. The following reports are available for City administrative
access in the SFS learning management system(AboveLMS).
These tools are available 24/7/365.
i. Products Purchased: When a purchase is made through the
City's SFS-provided web portal, the Products Purchased
report will reflect said purchase in real time.
2. Certificates Awarded: When the learner completes the Course
or Exam„the Certificates Awarded report will provided City
administrators the learner's name, completion date,pass/fail
status, certificate ID,and other:relevant information.
ii. An SFS client services representative will provide tutorials on how
to use these reports, as desired by the City.
