It is clearly understood that none of the services shall in any way imply or constitute the offering or
the providing of legal services or opinions. It is also understood that this agreement is the same as
the original contract with no charges unless specifc0ly stated below and it acts only as a
continuation of the original agreement for 12 month(s). Said original contract being styled and
identified as City Secretary Contract No.32481.
UNIMPLOYMENT COMPENSATION MANAGEMENT SERVICES
PART I - Claims Management
PART II - Education Management
PART III - Audit Management
PART IV - Acquisitions,Diverstitures and increased activity that occurs during this
contract period will be reviewed by CCC Management. CCC will discuss
these issues with the client prior to taking any type of corrective
modification.
CANCELLATION POLICY

Either the Client,orthe Company may terminate this agreement by giving the other party thirty
(30) day's advance notice. This Right is in addition to and not in lieu of any other termination
rights granted under the original contract.
This agreement shall be effective from 11/1/2007 to 10/31/2008. Client shall make semiannual
payments at the beginning of each six months in the amount of$2,100.00.
