FIRST EXTENSION OF CITY SECRETARY
CONTRACT NO 36477
THIS FIRST EXTENSION to City Secretary Contract No. 36477 (hereinafter "Extension") is
entered into by and between the City of Fort Worth("City"),a home-rule municipality in the State of
Texas,acting through Charles W. Daniels, its duly authorized Assistant City Manager,and Accutest
Laboratories Gulf Coast, Inc., a corporation registered in Texas, acting through
S ed6t ie's''its duly authorized President or Vice President, hereinafter called
"Contractor".
WHEREAS,the City and Contractor entered into that certain agreement identified as City Secretary
Contract No. 36477 (also the "Contract") commencing on November 1, 2007 and ending October
31, 2008 with two consecutive one year renewal terms for environmental laboratory analysis
services; and
WHEREAS,the City and Contractor wish to exercise the first extension option;
WHEREAS, the parties agree that all other conditions,terms and pricing shall remain the same as
stated in City Secretary Contract No. 36477.
NOW,THEREFORE,for and in consideration of the mutual covenants contained herein,and other
good and valuable consideration,the receipt and sufficiency of which are hereby acknowledged and
confessed, the parties hereto agree as follows:
The Contract with Accutest Laboratories Gulf Coast, Inc., for environmental laboratory analysis
services known as City Secretary Contract No. 36477 shall be extended pursuant to Article 1 "Term"
for its first one (1) year renewal beginning November 1, 2008 and ending October 31, 2009.
