CITYOEEORTWORTH PROFESSIONAL SERVICES AGREEMENT
CHYSECRETARY
This SECOND RENEWAL AND AMENDMENT OF FORT WORTH CITY
SECRETARY CONTRACT NO. 50002 is made and entered into by and between the City of
Fort Worth,a home-rule municipal corporation of the State of Texas ("City"), acting herein by and
through its duly authorized Assistant City Manager, and PetVet Relief Services, PLLC
("Contractor"), a Texas professional limited liability company, each individually referred to as a
"party" and collectively referred to as the "parties.".
WHEREAS, City and Contractor are currently parties to an agreement for veterinary
services, the same being Fort Worth City Secretary Contract No. 50002 which was executed on
November 27, 2017 with an effective date of October 24, 2017 (the "Agreement"); and
WHEREAS, the Agreement provides for a one-year initial term with options for up to
four, one-year renewal tenns; and
WHEREAS,on October 24,2018,the Agreement was renewed through October 23,2019;
and amended to provide for additional services; and
WHEREAS, the Parties now wish to renew the Agreement under the same terms and
conditions for a second one-year renewal term, commencing October 24, 2019; and
WHEREAS,the Parties also wish to amend the Agreement to again provide for additional
services, including a fee for in-heat spays, pursuant to the Agreement.
NOW, THEREFORE, the City of Fort Worth and Contractor do mutually covenant and
agree as follows:
1. RENEWAL
City and Contractor hereby agree to renew and extend Fort Worth City Secretary Contract
50002 for a second Renewal Term of one (1) year, beginning on October 24, 2019, and ending
October 23, 2020.

2. AMENDMENT
City and Contractor hereby agreement to amend the Agreement as follows:
Exhibit "A" Scope of Services. The itemized services included in the Agreement as Exhibit"A"
shall be amended to include in-heat spays and assessment of animals and as follows:
5. Contractor will have the ability to decline animals the Contractor determines are not good
surgery candidates at all times.
7. Contractor will perform vet checks - animal physical assessment of injury, disease or illness.
Vet check fees may be assessed per Exhibit`B"—Payment Schedule.
Exhibit `B" Payment Schedule. The payment schedule included in the Agreement as Exhibit
"B" shall be amended to allow for an animal assessment fee and to clarify the minimum amount
paid per surgery day as follows:

In-heat Spay Comprehensive spay for in-heat animal Additional

Dog or Cat
Contractor shall be paid a minimum of$600 per
surgery day. The$600 minimum shall be paid only
for those days that the City fails to provide at least
thirty eight(38) surgeries for Contractor to
complete. The $600 minimum will not be paid for
days in which the City provides at least thirty eight
(38) surgeries to complete and the Contractor fails to
complete the surgeries due to scheduling conflicts,
illness or any other reason.

All terms and provisions of City Secretary Contract 50002 shall remain in full force and
effect throughout the second Renewal Term unless specifically amended herein.
IN WITNESS WHEREOF, the said parties have hereto set their hands and seal of office
to triplicate originals on this 26th day of October , 2019.
