COUNTY OF TARRANT S
WHEREAS, the City of Fort Worth (City) and Stream Water Group
Inc. , (Engineer) made and entered into City Secretary Contract No.
30209, (the Contract) which was authorized by the City Council by
M&C C-20121 on the 22nd day of June 2004, and subsequently revised
by one previous amendment; and
WHEREAS, the Contract involves engineering services for the
following project:
Water and Sanitary Sewer Replacement Contract 2004 ; STM-D
WHEREAS, it has become necessary to execute Amendment No. 2 to
said Contract to include an increased scope of work and revised
maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through
their duly authorized representatives, enter into the following
agreement which amends the Contract:

Article I of the Contract is amended to include the additional
engineering services specified in a proposal letter dated March 20,
2008, a copy of which is attached hereto and incorporated herein.
The cost to City for the additional design services to be performed
by Engineer total $5,605.00 (Water Cost: $1,580.00; TPW Cost:
$4 ,025.00) .

Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall not
exceed the sum of $101,039.00.

All other provisions of the contract which are not expressly amended
herein shall remain in full force and effect.

Dear Mr.Chaubey:
The tollowing is a summary of additional work requested for Amendment No.2 to the Water and Sewer Replacement
Contract 2004 STM-D,per recent design meetings with City of Fort Worth officials.
The proposed amendment includes additional reproduction cost,survey and design,and modification of Plan Sheets.
Additional Survey for Atlanta St Curb and Gutter Design(100 If @ 3.50/1.f.) _ $1,400
Curb and Gutter Design Atlanta St(25 hrs@$105s) = $2,625
Additional Reproduction: 13 Sheets/set total 15sets with Mylar= $590
Modification of Existing Plan and Contract Documents
(Location Map, Cover Sheet, Bid Proposal Section)(4hrs@S105) = S 120
Modification ofSWPPP Documents(6hrs(a S95) = $570
The total cost to perform the additional services included in
Contract Amendment No.2 is= $5,605
The current status of the contract between Stream Water Group,Inc and City of Fort Worth for the Water and Sewer
Replacement Contract 2004 STM-D is as follows:
Original Contract Amount= $85,534.00
Amendment No. I Amount= $9,900.00
Amendment No.2 Amount= S5.605.00
Total Contract Amount including Amendment No.2= $101,039.00
Please process Amendment No.2,to the Water and Sewer Replacement Contract 2004 STM-D in the amount of
$5,605.00.
