COUNTY OF TARRANT §
WHEREAS, the City of Fort Worth (City) and Slater Engineering, Inc., (Engineer) made and
entered into City Secretary Contract No. 34076 (the Contract) which was authorized by the City
Council by M&C C-21723 on the 26th day of September, 2006; and
WHEREAS, the Contract involves engineering services for the following project:
2004 Capital Improvement Project Contract 45
Pavement Reconstruction and Water and Sanitary Sewer Main Replacement on
Spindletree Lane (Buttonwood Drive to Pepperbush Drive
Capital Project No. 00446; DOE No. 5399
Water Project No. P253-6041700446
Sewer Project No. P253-7041700446
TPW Project No. C200-2044000446
WHEREAS, it has become necessary to execute Amendment No. 2 to said Contract to
include an increased scope of work and revised maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through their duly-
authorized representatives, enter into the following agreement which amends the Contract:
1.
Article I of the Contract is amended to include the additional engineering services as
specified in a proposal letter dated August 30, 2007, a copy of which is attached hereto and
incorporated herein. The cost to City for the additional services to be performed by Engineer
total $ 9,160.00.
2.
Article II of the Contract is amended to provide for an increase in the maximum fee to be paid to
Engineer for all work and services performed under the Contract, as amended, so that the total
fee paid by the City for all work and services shall not exceed the sum of $ 134,288.00.

All other provisions of the Contract, which are not expressly amended herein, shall remain in full
force and effect.

Storm Drain Construction Plans
Dear Ms.Johnson:
Submitted herewith is Slater Engineering,Inc.'s(SEI)proposal for performing the additional engineering
services requested for Spindletree Lane.
SEI proposes to amend City Secretary Contract No. 34076, approved by Mayor and Council
Communications No 21723, and add certain engineering services to the scope of work. The additional
Scope of Services will provide for;
1 Engineering,Design and preparation of construction documents for the relief storm
drain through the limits of the paving on Spindletree Lane: and,
The additional services will result in additional costs for engineering, design, reproduction and computer
services. In additional the following increase in fees and services will be as follows:
Item No. 1. Add Parallel Relief Line Throughout paving limits.
A Revise Hydraulic Computations and Drainage Area Map
And Calculate HGL;
EIII 30 Hours @$150/Hr._ $ 4,500.00
B Engineer Relief Storm Drain Line,Inlet Lead Line;
Ell 16 Hours @ 135/Hr. $ 2,160.00
C Prepare Construction Plans;
Ell 20 Hours @$125/Hr._ $ 2,500.00
