CONTRACT No 1rNOOM,
SECOND AMENDMENT TO
CITY SECRETARY CONTRACT NO. 33797
WHEREAS, on July 27, 2006, the City of Fort Worth ("City") and Easter Seals
Greater Northwest Texas ("Easter Seals"), made and entered into City Secretary
Contract No. 33797, (the "Contract") which provided the opportunity to own a safe,
decent and affordable home to individuals with disabilities or families with a member
who has'disabilities;
WHEREAS, on October 11, 2007, City and Easter Seals made and entered into
City Secretary Contract No. 35968 (the "Contract Amendment") which amended the
contract to clarify certain terms and extend the time for performance;
WHEREAS, the term of the Contract as extended by the Contract Amendment
expires by its terms on January 25, 2008; and
WHEREAS, it is the mutual desire of City and Contractor to extend City Contract
No. 33797 for an additional six (6) months.
NOW, THEREFORE, City, whose address is 1000 Throckmorton St., Fort Worth,
Texas 76102, acting by and through T. M. Higgins, its duly authorized Acting Assistant
City Manager, and Easter Seals Greater Northwest Texas, whose address is 1555
Merrimac Circle, Suite 102, Fort Worth, Texas 76107, acting by and through Monica
Prather, its duly authorized President/CEO, do hereby agree as follows:

1. GENERAL PROVISIONS
B. Duration
The first paragraph of this section is amended to be and read as follows:
"The term of this Contract begins on the date of its execution and terminates July
24, 2008."
All other terms and conditions of City Secretary Contract No. 33797 as amended
by City Secretary Contract No. 35968 not amended herein remain unaffected and in full
force and effect, are binding upon the Parties, and are hereby ratified by the Parties.
