FIRST AMENDMENT TO
FORT WORTH CITY SECRETARY CONTRACT NO. 51935
This First Amendment to Fort Worth City Secretary Contract No. 51935 ("First
Amendment") is made between the City of Fort Worth ("Fort Worth"), a municipal corporation
and Jeff Cole Group, Ltd, an Ohio limited partnership ("Vendor").
WHEREAS, Fort Worth and Vendor entered into an Agreement'identified as-Fort Worth
City Secretary Contract No. 51935 beginning February 1, 2019 (the "Agreement"); and
WHEREAS, it is the collective desire of both Fort Worth and Vendor to amend the
Agreement to change the scope of services.
NOW THEREFORE,known by all these present, Fort Worth and Vendor, acting herein by
the through their duly authorized representatives, agree to the following terms, which amend the
Agreement as follows:
1. The Agreement is hereby amended by adding Exhibit A, attached to this First
Amendment, as a replacement for Exhibit A of the Agreement.
2. All other terms,provisions,conditions, covenants and recitals of the Agreement not
expressly amended herein shall remain in full force and effect.

SCOPE OF SERVICES
Following are planned services and costs for the Jeff Cole Group, Ltd.to provide to the City of Fort Worth,Texas.
TRAINING&COSTS:
2 Lean Yellow Belt Sessions(1 Spring 2020, 1 Fall 2020)
• Session 1 (up to 13 students): .................................................$13,500
• Session 2 (up to 13 students): ..............................................$13,500
• Students(14-25): .................. $750/per student
• Ex: 2 Classes of 25: Students 1-13: $13,500+(12 *$750)=$22,500 Total,x 2 Sessions=$45,000
Custom video development on Process Improvement as listed in Exhibit A:
• Development.............................................................................$200/hour
• Estimated that a typical 5-minute screencast video would take 8 hours($1600)
TOTAL TRAINING COST NOT TO EXCEED$60,000
PAYMENT TERMS:
Client to be invoiced after completion of each event. Payment terms are Net 30. Please make payment to: The

• Pre-course survey
• Pre and post-course videos
• Hard-copy Student binders
• Yellow Belt tools and templates
• Yellow Belt Exam, evaluation and reporting
• Yellow Belt certification
• Exercise materials
• (1)instructor including all travel expenses to Fort Worth,Texas
Custom Video Creation to Include
• Write video script
• Create custom powerpoints
• Provide voice-over or use CFW-provided audio file of narration
• Create and edit.mp4 process-improvement training videos ready for CFW to use internally
Ft.Worth to handle all costs and logistics associated with providing:
• Training facility (preferred layout to be provided by instructor)
• Projector and screen
• Flipcharts or whiteboards and markers
• Table and chair for instructor
• Any meals or refreshments for students
• Access to a photocopier

• Point of contact for logistics and receiving shipped student materials

• Any costs associated with ADA compliance for the event
