(Engineer)made and entered into City Secretary Contract No.33509(the"Contract")on the 19th day of May,2006; and WHEREAS,the Contract involves engineering services for the following project: WOODVALE SANITARY SEWER IMPROVEMENTS WHEREAS,it has become necessary to amend said Contract to: (1)Design and prepare preliminary and final plans for 5870 LF of waterline on Woodvale Road,Lon Morris Court, Vdali Court,Malaga Drive,and 250 LF of bore across Lake Worth. This work will include additional survey,modifying the contract documents to include bid proposal,specifications,and details for the waterlines and for the sewer line connection to manhole at Marina Drive Lift Station,as well as including the additional quality control and assurance. (2)Perform geotechnical investigation by soil bore and design sanitary meter station at Malaga Park.This work will include modifying the contract documents, addition of plan sheets and details for meter station and appurtenances and to perform quality control and assurance. (3)Revise the Contract's maximum fee. NOW THEREFORE,City and Engineer,acting herein by and through their duly authorized representatives,enter into the following agreement which amends the Contract: 1. Article I, Section 1 of the Contract is amended to include the additional engineering services specified in the Scope of Services,incorporated—herein as"Memorandum,Amendment No. L" The cost of the µ additional seD ices shall not exceed the sum of$96,008.00 jR CITY SECRETARY FT.WORM.Ty 2. Article II of the Contract is amended to provide for an increase in the maximum fee to be paid by City to Engineer as compensation for all work and services performed under the Contract,as amended, so that the total fee paid by the City for all work and services shall not exceed the sum of$203,738.00 1 ORIGINAL Compensation details for the revised scope and for the additional services outlined in this Amendment No. 1 are provided in the revised"Attachment A,Task Fee Estimate",attached hereto and incorporated within this Contract. 3. PROJECT DESCRIPTION The project site is located within Malaga Park which is located adjacent to and north of Malaga Drive in Lake Worth, Texas. The project consists of a proposed meter station. The meter station will consist of a 4 foot by 8 foot concrete meter vault. The bottom of the meter vault will be at a depth of 10 feet below grade. SCOPE OF WORK The geotechnical investigation performed for the referenced project will consist of field and laboratory investigations, engineering analysis, and a report prepared by a Registered Professional Engineer. i i Field Investigation The field investigation will consist of drilling one (1) test boring in the vicinity of the proposed structure. July 17, 2008 Page 2 Subsurface soil samples will be secured with thin walled tube and/or split spoon samples depending on soil type and consistency. Rock encountered in the borings will be evaluated using the Texas Department of Transportation Penetrometer (TxDOT Cone). All samples will be properly logged, packaged, sealed, and placed in a core box for transportation to the laboratory. Mas-Tek Engineering assumes that the client has the right-of-entry to the property and that the boring locations will be accessible to our conventional truck mounted drilling equipment during normal working hours. Should unusual soil conditions be encountered, we will call you with a recommendation and cost estimate to explore these unusual conditions. Mas-Tek Engineering will contact Dig Tess and the City of Lake Worth Water Utilities to have them locate underground utilities. However, Mas-Tek Engineering is not responsible for damage to underground utilities that are not identified prior to drilling. Laboratory Investigation Laboratory tests will be conducted to classify the soil and to evaluate the volume change potential and strength of the soil present at the site. Soil classification tests will typically consist of Atterberg limits (plasticity index), percent passing #200 sieve, moisture content, and dry unit weight. The volume change potential of the soils will be evaluated by swell tests. The strength of the soil will be estimated using hand penetrometer test and unconfined compressive strength testing. Engineering Analyses Results of field and laboratory work will be presented in an engineering repert. The report will include our recommendations to guide design and construction of the foundation and will include the following: 1. Plan of borings, boring logs, water level observations, and laboratory test results. 2. Foundation recommendations for the proposed meter station. 3. Comments on the presence and effect of expansive soils on slab-on-grade construction will be provided. Alternative methods of reducing any anticipated shrink/swell movements associated with expansive clays will be included for slab-on-grade construction, if required. 4. Recommendations for site grading and compaction of earthwork will be presented. GEOTECHNICAL FEES Based on the proposed scope of work, we will provide the geotechnical investigation for this project for a lump sum fee of$3,850.00. Turner, Collie &Braden, Inc. Proposal for additional services on Woodvale Sanitary Sewer and Waterline Improvements by Blackstone Engineering PLLC Scope of Work - Blackstone Engineering PLLC For Woodvale Sanitary Sewer Project 1. Design Phase Services- a. Preliminary Investigation-Subconsultant(Blackstone Engineering PLLC) will coordinate with the TCB's Project Manager/City of Fort Worth(as necessary),and franchise utilities to obtain as-built plans and record documents for utilities and streets,plats,right-of-way maps,existing easement information,contour maps,and other features within and pertaining to the Project. Subconsultant will perform one site visit to view the meter vault location and the pipeline route. b. Preparation of Preliminary Design-Subconsultant will perform the following tasks to prepare the preliminary design of the Project': i. Prepare plans for construction of approximately 750 linear feet of low pressure sewer line for the Woodvale Sanitary Sewer Improvements project, for inclusion with TCB's pipeline design plans. Plans for one below ground meter vault will be prepared,including mechanical,structural,electrical and instrumentation drawings. TCB will provide all drafting for civil,mechanical work for pipeline and valve vault plans. Electrical and structural plans will be prepared by consultants to Blackstone Engineering PLLC. ii. Establish plans,profiles,cross-sections and details of the low pressure sewer system. iii. Submit four sets of schematic level design(30%complete)to the TCB,for review.All preliminary sets of plans will be submitted in electronic(pdf)of half-size(11x17). iv. Meet with the TCB/City to discuss design comments. Incorporate comments into the schematic level design; V. Continue with design phase to 60% completion. vi. Submit four sets of plans to TCB/City for review. Meet with TCB/City to discuss comments. C. Preparation of Final Design-Subconsultant will perform the following tasks to progress the preliminary design of the Project to the final design level: i. Revise the preliminary(60% complete)drawings by incorporating comments from TCB/City ii. Finalize the drawings for the valve vaults and sanitary sewer line; Hi. Incorporate standard details into the drawing set and prepare additional details as required: iv. Prepare specifications for meter vault installation. V. Meet with the TCB/City to discuss the final drawings(100%complete)and associated quantity"takeoff'estimates. 2. Bid Phase Services- a. Preparation of Opinion of Probable Construction Costs-Subconsultant will prepare a final opinion of probable construction cost using the quantities defined by the final design. b. Assistance in Bid Advertisement Process-Subconsultant will assist TCB/City in advertising for bids. DISCUSSION: On May 2, 2006, (M&C C-21427) the City Council authorized the City Manager to execute an engineering agreement with TCB, Inc., in the amount of$107,730.00 to prepare plans and specifications for the Woodvale Sanitary Sewer and Water Extension Project. This agreement included the design of a low-pressure sanitary sewer system to bring sanitary sewer service to 110 residents on Woodvale, Malaga, Kay Lea Dawn Court, Vinca Circle, Lon Morris Court, and Vdali Court at Lake Worth. Currently, property owners must rely on privately owned on-site septic systems for their sewer service. The project involves the installation of approximately 9,400 linear feet of HDPE low-pressure sewer line with service lines from the intersection of Malaga and Cahoba near Mosque Point Park on the west end to the Town of Lake Worth lift station at Marina Drive on the east end. During discussions with the Town of Lake Worth on the connection to their lift station, they indicated a desire to have a sewer deduct meter installed. They also wanted to cease providing water service to Fort Worth residents in the Boy Scout Camp, Lon Morris Court, East Malaga and Vdali Court area. The Town of Lake Worth currently provides these 22 homes and the camp with water service through a series of one and two inch water lines that do not meet current Fort Worth standards for fire protection. Amendment No. 1 will provide the preparation of plans and specs for approximately 5,870 linear feet of eight inch waterline improvements, modification of the sanitary sewer bid documents to include the sewer meter station, waterline improvements, plans for water and sewer borings at Comanche Creek and a sewer manhole replacement prior to the connection to the Lake Worth system. TCB, Inc., proposes to provide these services for an additional lump sum fee of$96,008.00 to the original contract amount. City staff considers this fee to be fair and reasonable for the services proposed. TCB, Inc., is in compliance with the City's M/WBE Ordinance by committing to an additional 30 percent M/WBE participation on this Amendment No. 1. The City's goal on this amendment is 20 percent.
