SUBJECT: REQUEST TO FILE DOCUMENTATION TO CITY SECRETARY
CONTRACT NO. 30908
BACKGROUND
On March 10, 2020, the City Secretary's Office received Change Order No. 18 to City Secretary
Contract No. 30908 from Water Department. After research done by the Administrative Assistant it
was discovered that Change Order No. 17 was never filed. It was asked that the department provide
Change Order No. 17 to City Secretary Contract No. 30908. After some research from Environmental
Program Manager, Steven Nutter, it was discovered that this Change Order was not routed for
signatures and was never approved. Correspondence related to the location of the original and the
request for filing has been included in the contract file.
AUTHORIZATION FOR CONTRACT NUMBER
Per the direction of Mary Kayser, City Secretary, the City Secretary's Office has been requested to
issue a contract number, and place in the City of Fort Worth's official contract files Change Order
No. 17 that was missing.
This request has been approved.
