SUBJECT: REQUEST TO FILE AMENDMENTS MISSING TO CITY SECRETARY
CONTRACT NO. 51177
BACKGROUND
On March 27,2020,the City Secretary's Office received Amendment No.3 to City Secretary Contract
No. 51177. During routine research before filing, it was discovered that Amendment No. 1 and No.
2 was never filed with the City Secretary's Office. Administrative Assistant contacted the Water
Department to get a copy of Amendment No. 1 and 2 but was informed this never recorded in our
office and is attached to Amendment No. 3 for informational reference. Therefore, it is requested that
this memo serve as informational reference in order to facilitate the processing of subsequent
Amendments. To view Amendment No. 1 and No. 2 please refer to CSC No. 51177-A3.
Correspondence related to the location of the original and the request for filing has been included in
the contract file.
AUTHORIZATION FOR CONTRACT NUMBER
Per the direction of Mary Kayser, City Secretary, the City Secretary's Office has been requested to
issue a contract number, and place in the City of Fort Worth's official contract files the Amendment
No. 1 and 2.
This request has been approved.
