FIRST EXTENSION OF CITY SECRETARY
CONTRACT NO 35924
THIS FIRST EXTENSION to City Secretary Contract No. 35924 (hereinafter "Extension") is
entered into by and between the City of Fort Worth ("City"), a home -rule municipality in the State of
Texas, acting through Charles W. Daniels, its duly authorized Assistant City Manager, and Unified
Services of Texas, Inc., a corporation registered in Texas, acting through\'OCkX-G %,CL\ Z\lCj4)
its duly authorized President or Vice President, hereinafter called "Contractor".
WHEREAS, the City and Contractor entered into that certain agreement identified as City Secretary
Contract No. 35924 (also the "Contract") commencing on October 1, 2007 and ending September 30,
2008 with two consecutive one year renewal terms for testing petroleum storage tank integrity and
stage II system compliance; and
WHEREAS, the City and Contractor wish to exercise the first extension option;
WHEREAS, the parties agree that all other conditions, terms and pricing shall remain the same as
stated in City Secretary Contract No. 35924.
NOW, THEREFORE, for and in consideration of the mutual covenants contained herein, and other
good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged and
confessed, the parties hereto agree as follows:
The Contract with Unified Services of Texas, Inc., for testing petroleum storage tank integrity and
stage II system compliance known as City Secretary Contract No. 35924 shall be extended pursuant
to Article 4 "Times for Task Initiation and Completion; Contract Term" for its first one (1) year
renewal beginning October 1, 2008 and ending September 30, 2009.
