APPENDIX B-14
In accordance with the Professional Services Agreement signed between Matrix
Resources, Inc. ("Contractor") and the City of Fort Worth ("Client") on June 22, 2005,
City Secretary Contract (CSC) No. 32032, as amended by CSC No. 37230, collectively
the "Professional Services Agreement," this Appendix B-14 is incorporated into the
Professional Services Agreement and hereby reads as follows:
1. The term of this work order shall be extended to end when all services have
been completed under this work order as contemplated herein, unless
otherwise terminated in accordance with CSC No. 32032, as amended.
2. This Work Order incorporates by reference all of the terms and conditions of
the Professional Services Agreement, CSC No. 32032, as amended, and
specifically the services to be provided under Section 1 of said agreement.
3. The services to be performed under this Work Order shall be as follows:
HELP DESK AGENT (Computer Help Desk Environment)
To troubleshoot, modify and correct MS Exchange email box issues. To provide general
computer customer service support and information in operating, using and maintaining
computer systems and applications; to conduct various technical tasks related to the
operation and maintenance of information systems and equipment. To troubleshoot
hardware/software problems for failure determination. To create/modify/remove network
and mainframe security accounts.
Personnel assigned to provide services under this Work Order:
Name: Antonio Zamora
Start Date: July 20, 2009
End Date: Upon completion of all services contemplated herein, unless otherwise
terminated in accordance with CSC No. 32032, as amended.
Payment Terms:
Payment Rate: 30 per hour
