APPENDIX B-18
In accordance with the Professional Services Agreement signed between Matrix
Resources, Inc. ("Contractor") and the City of Fort Worth ("Client') on June 22, 2005,
City Secretary Contract (CSC) No. 32032, as amended by CSC No. 37230, collectively
the "Professional Services Agreement," this Appendix B-18 is incorporated into the
Professional Services Agreement and hereby reads as follows:
1. The term of this work order shall be extended to end when all services have
been completed under this work order as contemplated herein, unless
otherwise terminated in accordance with CSC No. 32032, as amended.
2. This Work Order incorporates by reference all of the terms and conditions of
the Professional Services Agreement, CSC No. 32032, as amended, and
specifically the services to be provided under Section 1 of said agreement.
3. The services to be performed under this Work Order shall be as follows:
Staff Augmentation for Gas Well Revenue Reporting
• The position requires MS Word, MS Access, MS Excel, SQL 2000 and 2005, SQL 2000
Enterprise Manager, SQL Server 2005 Management Studio, SSRS, SSIS, training and
documentation experience.
• The main duties consist of implementing Gas Well Revenue Reporting and a Reporting
Table Maintenance Application. Duties include analysis, conversion, development,
documentation, testing and implementation of all stored procedures, views, SSRS
reports, maintenance tables and the development of supporting applications and SSIS
jobs.
• This position will be responsible to the Financial Systems Administration Division of the
Financial Management Services Department for the operation, support, enhancement
and documentation of all systems reporting Gas Well Revenue data.
• The Planning and Development Department will provide funding and oversight of this
position.

Payment Terms.-
Payment Rate: $65.00 per hour
