AMENDMENT No.3
TO CITY SECRETARY CONTRACT No. 37238
WHEREAS, the City of Fort Worth (CITY) and Freese and Nichols, Inc . ,
(ENGINEER) made and entered into City Secretary Contract No. 37238, (the
CONTRACT) which was authorized by M&C C-22871 on the 7th day of June,
2008 in the amount of $ 177, 702; and
WHEREAS, the CONTRACT was subsequently revised by: Amendment Number
1 in the amount of $1, 102 , 424 which was authorized by M&C C-23507 on May
12 , 2009; Amendment Number 2 in the amount of $59, 821 which was
authorized by M&C C-24166 on May 13, 2010; and
WHEREAS, the CONTRACT involves engineering services for the
following project :
Clearfork Main Street Bridge, Project No. 00107; and
WHEREAS, it has become necessary to execute Amendment No. 3 to the
CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT:

Article I of the CONTRACT is amended to include the additional
engineering services specified in a proposal dated October 19, 2012 , a
copy of which is attached hereto and incorporated herein. The cost to
City for the additional design services to be performed by Engineer
totals $4 , 909 . (See Attached Funding Breakdown Sheet, Page -3-)

Article II of the CONTRACT is amended to provide for an increase in
the fee to be paid to Engineer for all work and services performed under
the Contract, as amended, so that the total fee paid by the City for all
work and services shall be an amount of $1, 344 , 856 .
3 .
All other provisions of the Contract, which are not expressly
amended herein, shall remain in full force and effect .

Clearfork Main Street Bridge: Proposal to Provide a plan sheet for the Clearfork-Hulen
Traffic Signal changes
Dear Mr. Wilson:
This letter is written in response to your request for Freese and Nichols (FNI) to prepare a
contract amendment for additional engineering services for the design of the Clearfork Main
Street Bridge project.
FNI will complete the following additional scope of services:
Pr,�o'eWct 5to��=ClearforkmHulen Traffic Signal/Ramp Changes
Develop one additional plan sheet for the intersection of Clearfork Main Street and Hulen
Street. This plan will show pedestrian ramps, pedestrian markings, signal poles, traffic control
cabinet, traffic signal heads, mast arms, and dimensions.
FNI and Savant Group will attend two (2) meetings with the City Traffic Group to discuss the
layout.
Deliverables
1. One PDF version of a 50% plan sheet and one PDF version of a 95% plan sheet for
discussions with the City of Fort Worth Traffic Group
2. Five (5) copies of 100% 11x17 final approved plan sheet.

Schedule
The 50% plan sheet will be submitted within one week of the notice to proceed. The 95% and
100% plan sheet will be submitted after receiving and addressing comments from the City
Traffic Group.
Compensation
FNI proposes to provide the above mentioned services for a lump sum fee of$4,909. This fee
includes a 10% markup of subconsultants and expenses as per the original contract Agreement.
