COUNTY OF TARRANT §
WHEREAS, the City of Fort Worth (City) and CP&Y Inc., (Engineer) made and
entered into City Secretary Contract No. 39059 (the Contract) which was authorized by
the City Council by M&C C-23748 on the 25th day of August, 2009, amended by City
Secretary Contract No. 39059-A1 on the 191h day of January, 2010 and amended by City
Secretary Contract No. 39059-A2 on the 24th day of March, 20100 and amended by City
Secretary Contract No. 39059-A3 on the 23`d of June, 2010 and amended by City
Secretary Contract No. 39059-A4 on the 28th of July, 2010 and
WHEREAS, the Contract involves engineering services for the following project:
Old Decatur Road from Bailey -Boswell to Old Boaz Road.
WHEREAS, it has become necessary to execute Amendment No. 5 to said
Contract to include an increased scope of work and revised maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through their duly
authorized representatives, enter into the following agreement, which amends the
Contract:
1.
Article I of the Contract is amended to include the additional engineering services
specified in a proposal letter dated April 4, 2011, a copy of which is attached hereto and
incorporated herein. The cost to City for the additional services to be performed by
Engineer total $6,600.00.
2.
Article II of the Contract is amended to provide for an increase in the maximum
fee to be paid to Engineer for all work and services performed under the Contract, as
amended, so that the total fee paid by the City for all work and services shall not exceed
the sum of $509,139.13.
3.
All other provisions of the contract, which are not expressly amended herein, shall
remain in full force and effect.

Dear Mr. Buckley:
The purpose of this letter is to request an amendment for additional services for miscellaneous design
revisions and additions requested by the City of Fort Worth. We respectfully submit the following items
for your consideration.
Additional Services: Right-of-way (ROW) staking
1. Provide lathes or paint marks along the proposed and existing ROW of Old Decatur Road. The
stake shall be set in close enough proximity to accurately depict the ROW for utility relocations
and to address landowner concerns.

Project Schedule
The project schedule will not be affected by the addition of the expanded scope and the project
completion date will remain unchanged.
Compensation
The cost for this amendment shall be an hourly fee not -to -exceed the figures listed here and in the
attached correspondence from Gorrondona.
ROW staking sub -consultant fee (Gorrondona) $ 6,000
10% fee for sub -consultant 600
Grand Total additional fee: $ 61600
The MWBE participation goal established for this project is 26%. MWBE participation in this additional
portion of the project will be $6,000, Per this amendment, total MWBE participation is 28.9%.

DECATUR ROAD (from W. Bailey Boswell Road to W. J. Boaz Road)
Dear Mr. Hague:
Gorrondona &Associates, Inc. (G&AI) is pleased to submit this proposal for
professional land surveying services for the above referenced project. The total
linear feet of surveying required is approximately 6,700 linear feet of proposed
paving and right-of-way improvements along Old Decatur Road. The following
itemized surveying services are to be provided by Gorrondona & Assoc. Inc.:
I. RIGHT-OF-WAY STAKING
1.) Provide lathes or paint marks along the proposed and exiting right-of-way
of Old Decatur Road. The stake shall be set in close enough proximity to
accurately depict the right-of-way for utility relocations and to address
landowner concerns.
Gorrondona &Associates, Inc. can complete the above itemized surveying tasks
for an hourly not -to -exceed fee of $6,000.00. If you have any questions or need
