COUNTY OF TARRANT §
WHEREAS, the City of Fort Worth (City) and CP&Y Inc., (Engineer) made and
entered into City Secretary Contract No. 39059 (the Contract) which was authorized by
the City Council by M&C C-23748 on the 25th day of August, 2009, amended by City
Secretary Contract No. 39059-Al on the 19th day of January, 2010 and amended by City
Secretary Contract No. 39059-A2 on the 241h day of March, 2010; and amended by City
Secretary Contract No. 39059-A3 on the 23`d of June, 2010 and
WHEREAS, the Contract involves engineering services for the following project:
Old Decatur Road fi•om Bailey -Boswell to Old Boaz Road.
WHEREAS, it has become necessary to execute Amendment No. 4 to said
Contract to include an increased scope of work and revised maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through their duly
authorized representatives, enter into the following agreement, which amends the
Contract:
1.
Article I of the Contract is amended to include the additional engineering services
specified in a proposal letter dated July 79 2010, a copy of which is attached hereto and
incorporated herein. The cost to City for the additional services to be performed by
Engineer total $4,239.00,
2.
Article II of the Contract is amended to provide for an increase in the maximum
fee to be paid to Engineer for all work and services performed under the Contract, as
amended, so that the total fee paid by the City for all work and services shall not exceed
the sum of $502,539,13,
3.
All other provisions of the contract, which are not expressly amended herein, shall
remain in full force and effect. __

Dear Mr. Buckley:
The purpose of this letter is to request an amendment for additional services for miscellaneous design
revisions and additions requested by the City of Fort Worth. We respectfully submit the following items
for your consideration.
Additional Services: Right-of-way (ROW) documents
1. Anew right-of-way document shall be created for the acquisition of land required for the traffic
signal at Boaz and Old Decatur.
2. 4 sets of documents shall be created for drainage easements in front of Bryson Elementary
School as a result of moving drop inlets off of City ROW.
3. The temporary construction easement (TCE) in front of Bryson Elementary school will be revised
to include additional room for grading as a result of moving the drop inlets off of City ROW.

Project Schedule
The project schedule will not be affected by the addition of the expanded scope and the project
completion date will remain unchanged.
Compensation
ROW Acquisition Sub -consultant fee (Gorrondona) $ 1,250
Drainage easement documents (4@$600ea) $ 21400
TCE revision $ 400
10% fee for sub -consultant 189
Grand Total additional fee: $ 41239
The MWBE participation goal established for this project is 26%. MWBE participation in this additional
portion of the project will be $4,050, Per this amendment, total MWBE participation is 28.13%.
