COUNTY OF TARRANT §
WHEREAS, the City of Fort Worth (City) and CP&Y Inc., (Engineer) made and
entered into City Secretary Contract No. 39059 (the Contract) which was authorized by
the City Council by M&C C-23748 on the 25th day of August, 2009;and which was
subsequently revised by Amendment 1 on the 19th day of January, 2010 administratively
authorized ($1,650.00); Amendment 2 on the 24th day of March, 2010 administratively
authorized ($10,010.00); Amendment 3 on the 23`d of June, 2010 administratively
authorized ($24,980.00); Amendment 4 on the 28th of July, 2010 administratively
authorized ($4,239.00); and Amendment 5 on the 12th of April, 2011 administratively
authorized ($6,600.00);
WHEREAS, the Contract involves engineering services for the following project:
Old Decatur Road from Bailey -Boswell to Old Boaz Road.
WHEREAS, it has become necessary to execute Amendment No. 6 to said
Contract to include an increased scope of work and revised maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through their duly
authorized representatives, enter into the following agreement, which amends the
Contract:
1.
Article I of the Contract is amended to include the additional engineering services
specified in a proposal letter dated June 275 20117 a copy of which is attached hereto and
incorporated herein. The cost to City for the additional services to be performed by
Engineer total $22,463.00.
2.
Article II of the Contract is amended to provide for an increase in the maximum
fee to be paid to Engineer for all work and services performed under the Contract, as
amended, so that the total fee paid by the. City for all work and services shall not exceed
the sum of $531,602.13.
3.
All other provisions of the contract, which are not expressly amended
remain in full force and effect.

Dear Mr. Buckley:
The purpose of this letter is to request an amendment for additional services for miscellaneous design
revisions and additions requested by the City of Fort Worth. We respectfully submit the following items
for your consideration.
Construction Phase Services
1. Provide services to the City during construction including, but not limited to answer RFIs and
making periodic site visits at the request of the City's Project Manager.
Project Schedule
The project schedule will not be affected by the addition of the expanded scope and the project
completion date will remain unchanged.

Compensation
The cost for this amendment shall be an hourly fee not -to -exceed the figures listed here.

Grand Total additional fee: $ 220463
The MWBE participation goal established for this project is 26%. MWBE participation in this additional
portion of the project will be $0. Per this amendment, total MWBE participation is 27.7%.
