WHEREAS, the City of Fort Worth (CITY) and Jacobs Engineering Group,
Inc, (ENGINEER) made and entered into City Secretary Contract No. 38733,
(the CONTRACT) which was authorized by M&C-23511 on the 12th day of May,
2009 in the amount of $251,818.00; and
WHEREAS, the CONTRACT was subsequently revised by: Amendment Number
1 in the amount of $48,553.00 authorized by the City Council by M&C C-
24033 on January 26, 2010, Amendment Number 2 in the amount of $19,969.00
administratively authorized by the City on May 26, 2010, Amendment Number
3 in the amount of $37,616.00 administratively authorized by the City on
September 30, 2010, and Amendment Number 4 in the amount of $0.00
administratively authorized by the City on June 27, 2011, and Amendment
Number 5 in the amount of $24,475.00 authorized by the City Council by
M&C C-25794 on August 21, 2012, and Amendment Number 6 in the amount of
$48,864.00 administratively authorized by the City on January 29, 2013;
and Amendment Number 7 in the amount of $7,854.00 administratively
authorized by the City on September 18, 2013; and Amendment Number 8 in
the amount of $17,814.00 administratively authorized by the City on
January 30, 2014; and
WHEREAS, the CONTRACT involves engineering services for the
following project.
North Beach Street from Vista Meadows Drive to south of Shiver Road;
WHEREAS, it has become necessary to execute Amendment No. 9 to the
CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT:

for
North Beach Street (Vista Meadows to Shiver)
City Project No. TPW=01292
SCOPE OF SERVICES
This supplement to the design services contract provides for north bound pavement rehab and
storm drain system "FB". Proposed pavement needs to be raised from approx Sta 94+00 to 99+50
approximately 8"-12" to allow for storm drain cross over existing 36" water line (Sta 95+75). Limits of
north bound pavement rehab are south of Statesman Rd to north of Heritage Glen (approx Sta
94+00 to 106+32).
1. North Beach Street Pavement rehab for northbound lanes from Sta 94+00 to 106+32

a. Match existing section horizontally
b. Use the previously approved pavement design for North Beach Street. A new
pavement design report is not included.

c) Quantity & Specs
a. Provide quantity and cost estimate
b. Provide standard CoFW spec book
d) Traffic Control Plan
a. Provide TCP plan and details to construct the north bound lanes
b. 4 sheets estimated, not including standards.
e) Paving Plan and Profile
a. Provide paving plan and profile for NB lanes
b. Raise profile for storm drain clearance, match existing grade for remainder of limits.
c. Use the previous survey/topo from North Beach Street project. A new topographical
survey is not included.
d. Include sidewalk for north bound paving limits. Sidewalk deferred from previous
project for south bound may be included at a later date.
e. 4 sheets estimated
f) Drainage Sheets
a. Provide hydraulic calculation sheets
b. Provide drainage area map
c. Provide plan/profile for storm drain system 'FB', as shown on exhibit.
d. Tie into existing jbox constructed with previous North Beach Street project.
e. Existing YMCA storm drain system to remain as -is.
f. Reconstruct existing inlet tops (Sta 95+75, 97+501 106+25)

a. Provide pavement markings and signage sheets per CoFW standards

3. Topographical Survey is not included, will use the existing topo from previous North Beach Street
project.
4. A new pavement design report is not included, will use the existing pavement design from previous
North Beach Street project.
