TO CITY SECRETARY CONTRACT
WHEREAS, the City of Fort Worth (CITY) and Jacobs Engineering Group,
Inc, (ENGINEER) made and entered into City Secretary Contract No. 38733,
(the CONTRACT) which was authorized by M&C-23511 on the 12th day of May,
2009 in the amount of $251,818.00; and
WHEREAS, the CONTRACT was subsequently revised by: Amendment Number
1 in the amount of $48,553.00 authorized by the City Council by M&C C-
24033 on January 26, 2010, Amendment Number 2 in the amount of $19,969.00
administratively authorized by the City on May 26, 2010, Amendment Number
3 in the amount of $37,616.00 administratively authorized by the City
Council on September 30, 2010, and Amendment Number 4 in the amount of
$0.00 administratively authorized by the City on June 27, 2011, and
Amendment Number 5 in the amount of $24,475.00 authorized by the City
Council by M&C C-25794 on August 21, 2012; and
WHEREAS, the CONTRACT involves engineering services for the
following project:
North Beach Street from Vista Meadows Drive to south of Shiver Road,
WHEREAS, it has become necessary to execute Amendment No. 6 to the
CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT:

Supplement #6
Dear Mr. Weiss:
Attached please find our scope and fee proposal for Supplement #6 to design services for North
Beach Street.' This scope includes culvert adjustments for Bewley Drive, Alta Vista tie-in to the
adjacent CoFW project, traffic control and sequencing modifications, and additional requested
median openings.

SUPPLEMENT #6 TO DESIGN SERVICES
for
North Beach Street (Vista Meadows to Shiver)
City Project No. TPW=01292
SCOPE OF SERVICES
This supplement to the design services contract provides for revisions to the Paving and Drainage
plans at Bewley Drive and Alta Vista.
I. Bewley Dr
1. Culvert Hydrology and update report
a. Due to the downstream development (Bewley Dr), update the hydraulic model to
ensure 'no -rise' of the flood plain. Per preliminary calculations, the culvert will need
to be lowered approximately 1'.
2. Culvert re -design
I
. Adjust the down stream headwall and grading due to meet the existing Bewley Dr.
b. Lower the flow line to meet the 'no -rise' criteria, as mentioned above
3. Roadway adjustment
a. Adjust the N. Beach Street profile to shift the low point south in order to avoid conflict
with existing Bewley Dr.
b. Adjust cross sections and earthwork quantity to match the profile revisions
4. Storm Drain re -design
a. Adjust storm drainage sheets to match new alignment (3 outfalls to culvert)
II. Alta Vista Dr
1. Adjust typical section
a. Change the typical section to match the proposed adjacent project to the east.
Revise section sheets to match
2. Adjust profile and limits
a. Adjust the profile to match the proposed adjacent project to the east. Revise profile
sheets to match.
b. Adjust the limits to match the proposed adjacent project to the east. Revise plan
sheets to match.
3. Adjust cross sections
a. Update/change the proposed cross sections to match the changes above.
4. Modify Traffic Control Plan (TCP)
a. Modify the TCP sequence/limits to match the proposed adjacent project to the east.
b. This includes full closure at Alta Vista during the summer time, this allows for '/2
construction per phase, versus the '/4 as currently designed.
c. Revise TCP sheets to match.
5. Modify the N Beach Street SB leftturn lane
a. Per request of CoFW, add 50' storage to the SB leftturn lane at Alta Vista. Revise
paving and removal sheets to match.
III. Add additional median openings
1. Add median opening south of Heritage Trace Parkway
a. Add median opening vicinity of Station 125+00, near Bewley Drive. Coordinate with
Hillwood Communities (Gena Terrell) for layout.
b. Add layout sheets/details as required. Adjust paving plans, TCP, removal plans, and
cross sections to match. Note that the existing NB dual left turn lane might need to
be adjusted.
c. It is assumed that no changes to the current profile of North Beach will be affected.
Any profile changes due to the median opening which affect drainage will be
considered additional services.

d. Add median opening vicinity of Station 135+00, between Heritage Trace Parkway
and Alta Vista. Coordinate with Hillwood Communities (Gena Terrell) for layout.

IV. Miscellaneous
1. Provide 5 full size bound plan sets for City use. Assume 300 sheets per set at
$1.50/sheet.

1. Any additional work not specifically included in the above scope of services will be accomplished
as Additional Services, after approval by the Client.
2. If additional meetings or additional meeting duration are required, they will be performed as
