WHEREAS, the City of Fort Worth (CITY) and Jacobs Engineering Group,
Inc, (ENGINEER) made and entered into City Secretary Contract No. 38733,
(the CONTRACT) which was authorized by M&C-23511 on the 12th day of May,
2009 in the amount of $251,818.00, and
WHEREAS, the CONTRACT was subsequently revised by: Amendment Number
1 in the amount of $48,553.00 authorized by the City Council by M&C C-
24033 on January 26, 2010, Amendment Number 2 in the amount of $19,969.00
administratively authorized by the City on May 26, 2010, Amendment Number
3 in the amount of $37,616.00 administratively authorized by the City on
September 30, 2010, and Amendment Number 4 in the amount of $0.00
administratively authorized by the City on June 27, 2011; and Amendment
Number 5 in the amount of $24,475.00 authorized by the City Council by
M&C C-25794 on August 21, 2012; and Amendment Number 6 in the amount of
$48,864.00 administratively authorized by the City on January 29, 20138
and Amendment Number 7 in the amount of $7,854.00 administratively
authorized by the City on September 18, 2013; and
WHEREAS, the CONTRACT involves engineering services for the
following project:
North Beach Street from Vista Meadows Drive to south of Shiver Road;
WHEREAS, it has become necessary to execute Amendment No. 8 to the
CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT:

SUPPLEMENT #8 TO DESIGN SERVICES
for
North Beach Street (Vista Meadows to Shiver)
City Project No. TPW=01292
SCOPE OF SERVICES
This supplement to the design services contract provides for revisions to the Paving and Drainage plans
due to waterline conflicts.
1. Raise southbound roadway profile between approximate Station 132+00 to Station
141+00 to clear two existing 36" Waterlines and one existing 24" Waterline. The
waterlines are substantially higher than the as-builts and have been potholed, survey locates
have been provided to Engineer. Per meeting on 10/28/13, maintain a minimum of 4' from top
of pavement (3.5' absolute minimum) to top of waterline.
a. Adjust Paving plan/profile sheets
i. Sheets 103, 104, 105, & 106
ii. Shift median opening from Sta 136+00 to Sta 138+00, due to elevation of existing
waterlines.
iii. A sag profile curve might need to be included to allow for cover, which would add
storm drain inlet and pipe.
b. Adjust Cross section sheet 34, 35, 36, 37
c. Adjust retaining wall sheets 113 & 114
d. Project Management/Coordination/Meetings
i. Project management, coordination as necessary and miscellaneous meetings
2. Adjust northbound profile and Inlet C3 to allow for direct drop inlet to Culvert 1.
a. Shift sag inlet C3 to approx Station 125+40, adjust profile as necessary.
b. Adjust sheets 102, 132, & 145 to reflect changes
3. Adjust 5-8'x4' MBC (Culvert 1) alignment (plan & profile) to miss 36" waterline, per field
pothole elevations of existing waterline.
a. Shift Culvert centerline 5' north
b. Raise profile approximately V
c. Design layout and wingwalls for pre -cast, which increases overall width of culvert
d. Adjust sheets 145, and 145a
4. Re -design Storm Drain SD-C, from Heritage Trace south to Culvert 1, to minimize
jack/bore operations.
a. Combine N/S crossing/jack/bore of Heritage Trace to one bore on the west side and
eliminate the east bore under Heritage Trace.
b. Add additional inlet and storm drain, if necessary, per profile adjustments for number 1
above.
c. Adjust sheets 132, 133, 134, and 135
5. Re -design Storm Drain SD=FB to miss 36" waterline in center and minimize storm drain
construction at YMCA parking lot. The existing 36" waterline in this location is substantially
higher than as-builts in this location.
a. Shift the storm drain crossing in the median to the north, next to the existing storm drain
crossing, to allow for storm drain over the 36" waterline.
b. Eliminate the proposed construction to the existing inlets in the YMCA parking lot in order
to minimize the construction impact to YMCA and utility conflicts. The existing inlets

currently tie into and existing storm system. This was originally proposed to separate
these inlets from the existing system and decrease the flow in that system.
c. Adjust sheets 141 and 142
6. Update Signal sheets at Alta Vista to reflect field changes per City direction, see sketch.
a. Redesign signal and ped poles per CoFW sketch/email (from Ken Brock, 10/14/13).
b. Adjust sheet 196

Shiver Road, Thereby Increasing the Total Contract Amount to $456,963.00 (COUNCIL

RECOMMENDATION:
It is recommended that the City Council authorize the execution of Amendment No. 8 to City
Secretary Contract No. 38733, an Engineering Services Contract with Jacobs Engineering Group,
Inc., in the amount of $17,814.00 for the design of North Beach Street from Vista Meadows Drive to
south of Shiver Road, thereby increasing the total contract amount to $456,963,00,
DISCUSSION:
This Agreement is for the design of North Beach Street from Vista Meadows Drive to south of Shiver
Road (City Project No. 01292) as a four -lane divided roadway. The proposed Amendment is for the
re -design of the southbound road profile between Alta Vista Road and Heritage Trace Parkway, and
adjust the culvert and storm drain to provide additional cover over shallow waterlines, and update the
traffic signal plan at Alta Vista Road. Funds for this Amendment will be provided from the 2008 Street
Improvements Fund.

Jacobs Engineering Group, Inc., proposes to complete Amendment No. 8 for a lump sum fee in the
amount of $17,814.00, for a revised total contract in the amount of $456,963,00. Staff considers this
fee to be fair and reasonable for the scope of services to be performed. These added design
services are needed in order to address unforeseen conditions encountered on the project during
construction.
This project is located in COUNCIL DISTRICT 4, Mapsco 22S, 221AI and 36A.
FISCAL INFORMATION/CERTIFICATION:
The Financial Management Services Director certifies that funds are available in the current capital
budget, as appropriated, of the 2008 Street Improvements Fund.
