AMENDMENT No. 4
CITY SECRETARY CONTRACT NO. 3 $13L/-9
WHEREAS, the City of Fort Worth (CITY) and Jacobs Engineering Group,
Inc., (ENGINEER) made and entered into City Secretary Contract No. 38733,
(the CONTRACT) which was authorized by the City Council by M&C C-23511 on
the 12th day of May, 2009 in the amount of $251,818.00; and
WHEREAS, the CONTRACT was subsequently revised by: Amendment Number 1

n the amount of $48,553.00 administratively authorized by the City on
January 26, 2010, Amendment Number 2 in the amount of $19,969.00
administratively authorized by the City on May 26, 2010, Amendment Number
3 in the amount of $37,616.00 administratively authorized by the City
Council on September 30, 2010;
WHEREAS, the CONTRACT involves engineering services for the following
project:
North Beach Street from Vista Meadows Drive to south of Shiver Road;
WHEREAS, it has become necessary to execute Amendment No. 4 to the
CONTRACT to include an increased scope of work and revised maximum fee;
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT:
1.
Article I of the CONTRACT is amended to include the additional
engineering services specified in the proposal letter dated May 11, 2011,
a copy of which is attached hereto and incorporated herein. There is no
cost to City for the additional design services to be performed by
Engineer.
2.
All other provisions of the Contract, which are not expressly amended
herein, shall remain in full force and effect.

Dear Mr, Weiss:
Attached please find our revised scope and fee proposal for Supplement #4 to design services
for North Beach Street. The revisions include the additional public open space and permanent
drainage easement document preparation, and the removal of pre -construction services from
the original contract scope. This supplemental will close out this contract.

SCOPE OF SERVICES
This supplement to the design services contract provides the property/right-of-way services for
additional permanent easement preparation. In addition, the supplement provides a revision to the
original contract to eliminate pre -construction services. This supplement will close out the contract.
PRE -CONSTRUCTION SERVICES
Exhibit "A-2" Part C —Pre-Construction Assistance of the original contract will be removed from the
scope of services. This includes the removal of the pre -bid assistance and the attendance of the
pre -bid conference, the bid opening and the pre -construction public meeting as well as printing cost.
The following items will be removed from the contract:
1. Public Meeting (Exhibit production and attendance)
2. Pre -Bid Assistance
3. Bid Opening
4. Pre Construction Conference
5. Printing —(partial cost)
II. RIGHT-OF-WAY SERVICES
1. PUBLIC OPEN SPACE DOCUMENT PREPARATION —Prepare five (5) public open
space easement documents that include a legal description with exhibit to City of Fort
Worth standards. An additional fee of $1000.00 per document will be required for
additional public open space easement document preparation.
2. DRAINAGE EASEMENT DOCUMENT PREPARATION — Prepare one (1) permanent
drainage easement document that includes a legal description with exhibit to City of Fort
Worth standards. An additional fee of $1000.00 per document will be required for
additional permanent drainage easement document preparation.

ASSUMPTIONS
1. Any additional work not specifically included in the above scope of services will be accomplished
as Additional Services, after approval by the Client.
