WHEREAS, the City of Fort Worth (CITY) and Jacobs Engineering Group,
Inc. (ENGINEER) made and entered into City Secretary Contract No. 38733,
(the CONTRACT) which was authorized by the City Council by M&C C-23511 on
the 12th day of May, 2009, and subsequently revised by Amendment Number 1
which was authorized by the City Council by M&C C-24033 on the 26th day
of January, 2010, and subsequently revised by Amendment Number 2 which
was authorized by the City on the 26th day of May, 2010;
WHEREAS, the Contract involves engineering services for the following
project:
North Beach Street from Vista Meadows Drive to south of Shiver Road;
WHEREAS, it has become necessary to execute Amendment Number 3 to said
Contract to include an increased scope of work and revised maximum fee;
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the Contract:
Article I of the Contract is amended to include the additional
engineering services specified in the proposal letter dated September 161
2010, copies of which are attached hereto and incorporated herein. The
cost to City for the additional design services to be performed by
Engineer total $37,616.00.

Article II of the Contract is amended to provide for an increase in
the maximum fee to be paid to Engineer for all work and services
performed under the Contract, as amended, so that the total fee paid by
the City for all work and services shall not exceed the sum of
$357,956.00.

All other provisions of the Contract, which are not expressly amended
herein, shall remain i hn full force and effect.

Attached please find our revised scope and fee proposal for Supplemental #3 to design services
for No, Beach Street. The revised concrete pavement design has been added to the
previously listed services including the additional storm drain analysis and as -built research
outside of the original survey limits, additional topographical survey, temporary and permanent
document preparation, and design for retaining walls on the west side of the roadway between
Heritage Trace Parkway and Alta Vista Drive.

North Beach Street (Vista Meadows to Shiver)
Property/Right-of-Way Services, Additional Survey
Additional Storm Drain Analysis and Design,
Additional Retaining Wall Design, and Revised Concrete
Pavement Design
City Project No. TPW-01292
SCOPE OF SERVICES
This supplement to the design services contract provides additional analysis and design services for
storm drainage due to a portion of the existing drainage system crossing previous topographical limits.
Additional survey is required in order to verify the existing conditions. This supplement also addresses
the property/right-of-way services for temporary and permanent easement preparation and the design
services for the added retaining wall between Heritage Trace Parkway and Alta Vista Drive. In addition,
the supplement provides revisions to the concrete pavement design based on the final draft of the City
of Fort Worth Pavement Design Manual dated June 2010.
I. PROPERTY/RIGHT-OF-WAY SERVICES
1. TEMPORARY EASEMENT DOCUMENT PREPARATION —Prepare twenty eight (28)
temporary construction easement documents that include a legal description with exhibit to
City of Fort Worth standards. An additional fee of $700.00 per document will be required for
additional temporary construction easement document preparation.
2. PERMANENT EASEMENT DOCUMENT PREPARATION —Prepare three (3) permanent
drainage easement documents that include a legal description with exhibit to City of Fort
Worth standards. An additional fee of $1000.00 per document will be required for additional
permanent drainage easement document preparation.
II. TOPOGRAPHIC/DESIGN SERVICES
1. TOPOGRAPHIC/DESIGN SURVEY —Provide additional topographic survey of an area
approximately 450' by 50' along the south side of Block 2 of the Heritage Glen Addition, from
the proposed west right-of-way line of North Beach Street to approximately 100' beyond the
headwall at the end of the 48" storm drain.
2. DELIVERABLES — Prepare a final digital topographic/design survey drawing in Microstation
format at a scale of 1 "=50'. The drawing shall show all features located and existing visible
utilities along with a Digital TIN and one -foot interval contours. Other deliverables shall
include a copy of all field notes and field sketches, a hard copy coordinate list of points
located in the field and a digital ASCII point list.
III. STORM DRAIN ANALYSIS S DESIGN
A. Design
1. Obtain information for inlet south of Heritage Glen Phase 1A subdivision.
a. Locate and research as -built plans.
b. Identify existing easements.

2. Extend storm drain design to inlet location.
B. Meetings
1. This scope of services includes two (2) meetings with the City to coordinate, explain, and/or
justify the hydrologic and hydraulic analysis. Meeting duration is assumed to be less than
two (2) hours per meeting.
2. This scope of services assumes a maximum of twenty (20) hours for coordination with the
client.
IV. RETAINING WALL DESIGN
A. Design
1. Analyze roadway design and identify locations of retaining walls.
2. Revise cross sections to include retaining walls.
3. Locate applicable retaining wall standards.
V. CONCRETE PAVEMENT DESIGN
A. Geotech
1. Revise Pavement Design Report based on the final draft of the City of Fort Worth Pavement
Design Manual dated June 2010.
B. Design
1. Revise typical section and quantities in plan set.

SUPPLEMENT TO AGREEMENT FOR SERVICES
SCOPE OF SERVICES AND FEES
This SUPPLEMENT to AGREEMENT FOR SERVICES to the original Agreement for Services (original Agreement dated August 21, 2009,
Agreement reference number P94090754 (Revised) is between Jacobs Engineering ("Client") and Terracon Consultants, Inc. ("Consultant") for
additional or changed Services to be provided by Consultant for client on the Project, as described In the Agreement for Services, unless otherwise
described in the Project Information section of Consultant's Supplemental Proposal dated September 17, 2010 ("Supplemental Proposal"), or Exhibit A
to this Supplement (which section or exhibit is incorporated into the Supplement). This Supplement is incorporated into and part of the Agreement for
Services.
1. Scope of Services. The scope of the additional or changed Services are described in the Scope of Services section of the Consultant's
Supplemental Proposal, unless Services are otherwise described below or in Exhibit B to this Supplement (which section or exhibit are
incorporated into the Supplement).
Terracon will re-evaluate the pavement design based on the City of Fort Worth Pavement Design Manual preliminary
draft dated June 2010, A letter report will be issued two weeks from date of authorization.
2. Compensation. Client shall pay compensation for the additional or changed Services performed at the fees stated in the Supplemental
Proposal unless fees are otherwise stated below or in Exhibit C to this Supplement (which section or exhibit are incorporated Into the
Supplement).
The cost will be $950.
All terms and conditions of the Agreement for Services shall continue in full force and effect. This Supplement is accepted and Consultant is
authorized to proceed.
