CONTRACT N0.39248
WHEREAS, on September 29, 2009, the City of Fort Worth ("City") and Cornerstone
Assistance Network ("Contractor") made and entered into City Secretary Contract No. 39248
(the "Contract");
WHEREAS, the Contract provided funding to Contractor pursuant to a grant received by
City from the United States Department of Housing and Urban Development ("HUD") under
Title XII of the American Recovery and Reinvestment Act of 2009, ("Recovery Act") for
utilization in connection with its Homeless Prevention and Rapid Re -Housing Program
("HPRP").
WHEREAS, the First Amendment reallocated HPRP funds within eligible cost items in
order to more accurately reflect the utilization of funds by the Contractor; and
WHEREAS, on December 13, 2011, the Fort Worth City Council approved M&C C-
25345 authorizing a contract amendment with Contractor to reallocate Sixty Five Thousand Five
Hundred Sixty Nine and 02/100 Dollars ($65,569.02) in HPRP funds from another contractor to
Contractor's Homelessness Prevention Program; and
WHEREAS, Contractor requires additional time under the Contract to utilize these funds;
and
WHEREAS, City and Contractor wish to streamline the process by which line items in
the budget can be modified to meet the needs of the program; and
WHEREAS, it is the mutual desire of City and Contractor to amend the Contract to meet
Contract objectives.
NOW, THEREFORE, City, whose address is 1000 Throckmorton St., Fort Worth TX
76102, acting by and through Fernando Costa, its duly authorized Assistant City Manager, and
Contractor, whose address is 3500 Noble Avenue, Fort Worth, TX, acting by and through Roddy
Youree, its duly authorized Chief Operating Officer, do hereby agree as follows:

Worth ("Program"), for the term September 1, 2009 to June 30, 2012."
b. Section 2.g. is hereby added to the Contract as follows:

"City will disburse additional HPRP funds in an amount up to SIXTY FIVE
THOUSAND FIVE HUNDRED SIXTY NINE AND 02/100 DOLLARS
($65,569.02) to Contractor on a reimbursement of expenses basis. Contractor is
acting as a subrecipient for the purposes of this contract in carrying out the
eligible activities as set forth in Exhibit A -Scope of Services"
c. Section 2.c. is hereby deleted and replaced with the following:
"HPRP Funds will be reimbursed in accordance with Exhibit D —Program
Operating Budget. During the Term, Contractor may submit written proposals
to increase or decrease line -item amounts in the Budget; such approvals are
subject to approval by the director of the City's Housing and Economic
Development Department ("Director"). If Director gives written approval of
Contractor's proposed Budget amendment (as approved, the "Amended Budget"),
then the Amended Budget will take effect on the first day of the month following
the month in which it was approved by Director. No Amended Budgets will be
approved after May 15, 2012. Information in Exhibit D — Program Operating
Budget (as amended) other than the line item budget on the first page is
informational and was created solely for purposes of preparing, negotiating, and
determining the reasonableness of the overall line item budget on the first page of
Exhibit D — Program Operating Budget and is not to be considered part of the
terms and conditions of this Contract. Contractor may make changes to these
details without the City's consent so long as the total amounts charged to the grant
do not exceed the line items on the first page of Exhibit D — Program Operating
Budget (as amended). Notwithstanding this language, Contractor may not make
changes to the column labeled "Position" without the prior written consent of the
City."
d. The attached Exhibit D —Supplemental is hereby added to the contract. All references
in the Contract to Exhibit D — Program Operating Budget as amended) apply to
Exhibit D — Supplemental.
e. This amendment is effective September 1, 2009.
II.
a. All other terms and conditions of the Contract not amended herein remain unaffected and
in full force and effect, are binding on the Parties and are hereby ratified by the Parties.
