AMENDMENT TO CITY SECRETARY
CONTRACT NO.39248
WHEREAS, on September 21, 2009, the City of Fort Worth ("City") and Cornerstone
Assistance Network ("Contractor") made and entered into City Secretary Contract No. 39248
(the "Contract");
WHEREAS, the Contract provided funding to Contractor pursuant to a grant received by
City from the United States Department of Housing and Urban Development ("HUD") under
Title XII of the American Recovery and Reinvestment Act of 2009, ("Recovery Act") for
utilization in connection with its Homeless Prevention and Rapid Re -Housing Program
("HPRP");
WHEREAS, Contractor desires to reallocate HPRP funds within eligible cost items by
increasing Salaries, FICA, Medicare, Health Insurance, and Office Supplies; but leaving the total
grant award at One Hundred Ninety One Thousand Five Hundred Ten Dollars and Seventy Five
Cents ($191,510.75) by reducing Rental Assistance, Security Deposits, Utility Payments,
Telephone and Private Auto Allowance; and
WHEREAS, it is the mutual desire of City and Contractor to amend the Contract to meet
Contract objectives.
NOW, THEREFORE, City, whose address is 1000 Throckmorton St., Fort Worth TX
70102, acting by and through Susan Alanis, its duly authorized Assistant City Manager, and
Contractor, whose address is 3500 Noble Avenue, Fort Worth, TX, acting by and through the
Roddy Youree, its duly authorized Chief Operating Officer, do hereby agree as follows:

a. Exhibit D is deleted in its entirety and replaced with the attached Exhibit D —Revised.

Leann,
Cornerstone has asked to revise their HPRP budget again, right after they executed the last amendment but
before we had executed it. So instead of executing that amendment, we will execute this one instead.
Like the CDBG, ESG and HOPWA amendments we recently discussed, I added language about what is being
increased and decreased.
Please let me know if you have comments or questions or if this is ready to go.
