NATIONAL INSURANCE CRIME RURFAU
EXTENSION OF VEHICLE USE AGREEMENT
The National Insurance Crime Bureau ("NICB") and the City of Fort Worth ("Fort Worth")
entered into a Vehicle Usage Agreement (Fort Worth City Secretary Contract No.
40323 dated June 11, 2010) in which NICB leased to Fort Worth the vehicle listed
below for the purpose of investigation of insurance -related crime and/or fraud.
Year:
WHEREAS, the terms of the Original Agreement were two years, effective the
date the vehicle was delivered into the custody of the Fort Worth Police Department.
WHEREAS, NICB and Fort Worth wish to renew the Original Agreement for an
additional two year period from 6/21/2012 to 6/21/2014,
NOW THEREFORE, in consideration of the mutual agreements set forth in the
Original Agreement, other good and valuable consideration, and the conditions and
agreements set forth below, the parties agree as follows:
1. The parties agree that the Original Agreement shall be renewed for a two
year period from 6/21/2012,
2. All other terms and conditions of the Original Agreement, as amended,
remain unchanged and in full force and effect.
The parties hereto have executed this Agreement on the 21st day of June, 2012.
