HomeMy WebLinkAboutContract 53166-A1 CSC #53166-A1 G��0• FIRST AMENDMENT TO CITY SECRETARY CONTRACT NO.53166 This Amendment("Amendment") is entered into by and between the City of Fort Worth("City") and the Girls Incorporated of Tarrant County Agency") by and through their respective duly authorized agents.City and Agency may be referred to individually as"Party"and collectively as"Parties." WHEREAS, City and Agency made and entered into City Secretary Contract No. 53166 ("Contract")on December 5,2019; WHEREAS, the Contract provides funding to Agency pursuant to a grant received by City from the United States Department of Housing and Urban Development ("HUD") through the Community Development Block Grant Program ("CDBG"), Program No. B-19-MC-48-0010, Catalog of Federal Domestic Assistance No. 14.218; WHEREAS, it is the mutual desire of City and Agency to amend the Contract to change the program temporarily due to COVID-19; Whereas, Agency requested temporary changes to the program due to the COVID-19 Pandemic. The Agency will use multiple forms of digital media for communication with clientele. Essential care packages will be provided to clients in need. See program changes in detail on Exhibit A —Program Summary; WHEREAS,City believes that the requested amendment will help Agency meet the program's goals and further the interests of both Agency and City;and WHEREAS,it is the mutual desire of City and Agency to amend the Contract to complete the Program and meet Contract objectives. NOW,THEREFORE,City and Agency hereby agree to amend the Contract as follows: I. EXHIBIT"A"—Program Summary attached to the Contract is hereby amended and replaced it1 its entirety with the EXHIBIT"A"—Program Summary Revised May 26,2020,attached hereto. II. This amendment is effective as of the date of execution of this Amendment. III. All terms and conditions of the Contract not amended herein remain unaffected and in full force and effect,are binding on the Parties and are hereby ratified by the Parties. Capitalized terms not defined herein shall have the meanings assigned to them in the Contract. WORTH,TX Page 2 Girls Incorporated of Tarrant County(CDBG) GIRLS INCORPORATED OF TARRANT COUNTY EXHIBIT "A" Education, Prevention, and Leadership Program PROGRAM SUMMARY PROGRAM SUMMARY (CDBG) October 1, 2019 to September 30, 2020 $70,000.00 PERIOD AMOUNT Capitalized terms not defined herein shall have meanings assigned them in the Contract. PROGRAM: Program provides in-school,after-school,and summer education and activities to improve life skills,promote academic success, and prevent juvenile crime and delinquency for girls, ages 13-19 years of age. Curricula that will be used include, Leadership & Community Action, Academic Advisement and Mentoring, Girls Beyond Bullying,Friendly PEERsuasion,and Economic Literacy. In the current events of school and public entity closures due to Covid-19,in person groups currently not being held. Online communication and virtual programs will be utilized until social distancing is lifted. Changes are to be considered temporary. Agency anticipates sharing their program information amongst the community in order to expand their services. Agency will work with schools to connect with students on their campuses. Agency will provide families with essential care packages to help meet their critical needs, including food, feminine hygiene products, and household goods. Note: Services were provided in person at the locations listed below, but are currently only offered in a virtual format until locations are able to open to serve clients in person. CDBG Funds will be used to pay for a percentage of salaries only. The percentage of any Program employee's salary will not exceed the percentage of Fort Worth clients participating in the Program and shall be cost allocated in accordance with the amount applicable to the CDBG funded Program. Agency's office is located at 1226 E. Weatherford Street, Fort Worth, TX 76102. Costs incurred at any locations that have not been approved by City shall not be reimbursed. Agency is a Fort Worth Agency which means at least 90% of clients served by the Program are Fort Worth residents. Agency will submit documentation which verifies that at least 51% of all clients served by the Program, whether Fort Worth residents or not, are Income Eligible Clients with current household incomes at or below 80% of AMI. REGULATORY CLASSIFICATION: IDIS Matrix Code(s) and Service Category: 05D Youth Services National Objective Citation: LMC Low/Mod Limited Clientele 24 CFR 570.208(a)(2)Regulatory Citation(s): 24 CFR 570.201(e) PROGRAM GOALS: Minimum Fort Worth Agency Program Performance Outcome: At least 51% of all Program clients must be Income Eligible Clients. Agency must furnish City a Certification of Income Statement for all clients served. For example, if the Program serves 1,000 clients, 510 must be Income Eligible Clients. In this example,Agency would be required to furnish City 1,000 Certification of Income Statements that prove at least 510 of the clients served by the Program are Income Eligible Clients. Minimum Number of Clients to be Served: The Program must serve a minimum of 233 Unduplicated Clients as shown by the monthly reports on Attachment 111. When determining the number of clients which constitutes 51%, HUD regulations require that partial numbers must be rounded up.
