HomeMy WebLinkAboutContract 53169-A1 CSC No.53169-A1 CG�ys�` FIRST AMENDMENT TO CITY SECRETARY CONTRACT NO.53169 This Amendment("Amendment")is made and entered into by and between the City of Fort Worth ("City")and Sixty and Better Inc.("Agency")by and through their duly authorized agents.City and Agency may be referred to individually as"Party"and jointly as"Parties." WHEREAS, the City and Agency made and entered into City Secretary Contract No. 53169 ("Contract")on December 05,2019; WHEREAS,the Contract provides funding to Agency pursuant to a grant received by City from the United States Department of Housing and Urban Development ("HUD") through the Community Development Block Grant Program ("CDBG"), Program No. B-19-MC-48-0010, Catalog of Federal Domestic Assistance No. 14.218; WHEREAS,itis necessary due to the COVID-19 Pandemic to allow Agency to modify its program from congregate transportation to a meal delivery service program to older adults displaced by the closure of Fort Worth Community Centers during the Pandemic; WHEREAS,the Agency requested a reduction in unduplicated clients by 60 because Agency projects only being able to serve 250 unduplicated clients due to the transition from one program to anothelr. WHEREAS,City believes that the requested amendment will help Agency meet a vital need in toe City's elderly population and further the interests of both Agency and City;and WHEREAS, it is the mutual desire of City and Agency to amend the Contract to provide this necessary service and meet the goal of providing critical services to elderly citizens who are in need at this time while furthering the National Objective. NOW,THEREFORE,City and Agency hereby agree to amend the Contract as follows: I. A. Section 5.3.3 shall be amended and replaced in its entirety with the following: Failure to meet at least 80%of its performance milestones or serve 250 Unduplicated Clients under this Contract may, in the City's sole discretion, disqualify Agency for consideration under the City's Request for Proposals for the 2020-2021 Program Year for federal grant funds. B. EXHIBIT"A"—Program Summary attached to the Contract is hereby amended and replaced in its entirety with the EXHIBIT"A"—Program Summary Revised June 29,2020,attached hereto. II. This Amendment is effective as of the effective date the Contract. OFFICIAL RECORD III. DBA SENIOR EXHIBIT "A" CITIZEN SERVICES Connecting Seniors to Community PROGRAM SUMMARY i PROGRAM SUMMARY (CDBG) October 1,2019 to September 30,2020 $115,000.00 PERIOD AMOUNT Capitalized terms not defined herein shall have meanings assigned them in the Contract. PROGRAM: CDBG funds were originally allocated to provide door-to-door transportation for Fort Worth seniors age 62 and over to attend older adult activity centers and for shopping or field trips. COVID-19 significantly changed this service. To slow the spread of the virus, Sixty and Better closed all older adult activity centers. When the Centers closed, many participants lost a vital source of their daily nutrition and critical socialization opportunities. To ensure participants continued to receive nutritious meals and have opportunities to stay connected during Center closures, Sixty and Better transitioned their congregate meal program into a meal delivery program,where meals are delivered to participants' homes.Deliveries will be made every other Monday between the hours of 10:30 AM and 2:00 PM. Each delivery includes 10 meals and each meal meets one-third recommended dietary intake for older adults. The program will continue as such for the reaminder of the contract. CDBG Funds will be used to pay for a percentage of salaries for the Chief Operations Officer. CDBG Funds will also be used to pay for meal deliveries on a per-meal basis. The cost for deliveries shall be the same as the rate charged to the Agency by the vendor or subcontractor delivering the meals. Meals will be picked-up by transit providers at the Sixty and Better Central Kitchen (5051 Resource Dr.,Fort Worth,TX 76119), and delivered to participants' homes. Only individuals who have been displaced from one of the following older adult activity for mealy centers are g delivery.
