cs In accordance with the Professional Services Agreement entered into between Lone

t 4.tStattorporate Services ("Contractor") and the Cityof Fort Worth("City")effective on or
about December 4, 2012, City Secretary Contract ("CSC31) No. 44153, (collectively the
"Professional Services Agreement"), this Appendix A-10 is incorporated into the
Professional Services Agreement and hereby reads as follows:
1. The term of this Work Order shall be extended to end when all services have
been completed under this work order as contemplated herein, but not later
than the end date set forth below.
2. This Work Order incorporates by reference all of the terms and conditions of
the Professional Services Agreement.
3. The services to be performed under this Work Order shall be as follows:
IT SENIOR PROGRAMMER ANALYST m ERP PHASE II
1. Support the ERP PeopleSoft Financial Project as necessary with specific
short-term engagements, including:
Analyze information processing requirements and systems
Translate requirements defined during analysis into logical, economical,
and practical system design alternatives
® Implement City approved alternative solutions
2. Troubleshoot, determine and resolve problems supporting migration activities
and/or operational processes
3. Participate in the maintenance and modification of current systems as needed
4. Assist City staff as required in using the Financial Processing and Reporting
computer systems operated by the City.
5. Write system documentation for staff and clients
6. Perform other related duties and responsibilities as required

Payment Terms:
Payment Rate: $92.00 per hour, inclusive of expenses

This Appendix A-10 shall terminate immediately in the event the contract
personnel does not begin to provide contract services to the City under this
Appendix as of the Start Date stated above. If the contract personnel begins
to provide services, but Contractor provides notice to the City that the contract
personnel will no longer provide services under the Appendix, and such
notice is effective prior to the End Date stated above, then this Appendix shall
terminate immediately as of the effective date of the notice from Contractor
