Worth Police Department, 1000 Calvert Street, Ft. Worth TX 76107 on January 7-8, 2013 for up to 100 attendees for the amount of $15,500 which includes all expenses except ground transportation between the airport, hotel and training site for two instructors during their stay, which the Ft. Worth Police Department will provide. The training will be conducted at the above Calvert Street address. The program will be conducted between 8:30AM — 4:30PM local time and will include a one -hour lunch break and up to four additional short breaks that day. Responsibilities of purchasing agency/organization It is understood that the Ft. Worth Police Department will provide or make arrangements for the following: • A room will be provided that will comfortably seat all attendees in a fashion that allows for clear sight of the instructor at all times. Each student will be provided with a writing surface (ideally a desktop) that will allow for note taking throughout the course. • The classroom will be equipped with a built-in sound system that allows for high quality, consistent voice and computer -displayed video sound projection. This system will allow for volume variation and stereo sound. A LAPEL MICROPHONE system, compatible with the house sound system, along with sufficient battery supply to last throughout the two-day course, will be provided. • An A/V technician or staff member thoroughly versed in all elements of the audio and visual systems of the main classroom will be made readily available for set-up prior to the start of the program and throughout the two days of training to ensure appropriate functioning of all A/V systems at all times. © The classroom will have a lighting system that allows for variable light levels and will be free from large, uncovered windows or other lighting challenges that would compromise the clarity of projection on the screen. o One newer model LCD projector, less than five years old, that provides a minimum projection strength of 2000 lumens and is set to function at full capacity (not on a lower -powered energy - saving setting) will be provided. The projector will be appropriately positioned in the room to provide for maximum image clarity as indicated by the "throw distance" specifications for the projector. o At the time of the start of the course, this main projector should contain either a new bulb or one that is functioning at new or near -new capacity. A back-up bulb should be readily available in the event of burnout or malfunction. Please note that powerful projection strength sufficient to provide for extremely clear still slide images and video footage viewing is critical to the successful presentation of the program. • A main screen of sufficient size and positioning to allow for all in the classroom to see clearly will be provided. • The venue will allow for the instructors' computers to be within their easy reach throughout the program. • It is the sole responsibility of the purchasing agency/organization to solicit attendance for this class. The Force Science Institute will, if requested, provide to the agency/organization class information for a promotional flyer or other promotional elements, but cannot participate in additional marketing efforts. 1 2- 4-1 2 P 1 2: 20 IN OFPYCYpY, RECORD CITY SECRETAPY Ft worm, A9S-17 979A97 18 Payment An invoice for $7,750, representing a non-refundable down payment of 50% of the total fee, has been issued to the Ft. Worth Police Department with this contract. It is understood that payment of this invoice is due on or before December 15. An invoice for the remaining 50% of the fee will be submitted immediately following the program and will be due upon receipt. Cancellation policy Upon signing, this course will be considered fully secured and may not be cancelled unless as the result of issues beyond the control of the Force Science Institute and the Ft. Worth Police Department that prevent the training program from being delivered. Said reasons will be limited to a phenomenon of nature of catastrophic proportions or intensity; extreme illness, hardship or unforeseen travel issues on the part of the instructors (such as unexpectedly cancelled flights that cannot be rebooked within the time frame designated for the course) that are unavoidable and out of their control; executive orders of the President related to national defense or congressional or state acts that prohibit the training program from being conducted. In the instance of an unavoidable cancellation, all possible efforts will be made to reschedule the class as soon as possible.
