AMENDMENT No. 1
TO CII' II'Y SECRETARY C NTRACT
WII-IERJEA@9 the City of Fort Worth (CITY) and BDS Technologies,
Inc., (ENGINEER) made and entered into City Secretary Contract No
43953, (CONTRACT) which was authorized by M&C C=25944 on the 23rd day
of October, 2012 in the amount of $]31,637OO; and
WHEREAS, the CONTRACT involves engineering services for the
following project:
Sanitary Sewer Rehabilitation Contract 88; and
WHEREAS, it has become necessary to execute Amendment No. 1 Lo
the CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ENG T NE R, acting herein by and through
their duly authorized representatives , enter into the following
agreement , which amends the CONTRACT:
1.
Article 1 of the CONTRACT is amended to include the
additional engineering services specified in a proposal dated
November 4, 2013, a copy of which is attached hereto and incorporated
herein. The cost to City for the additional design services to be
performed by Engineer totals $31, 230 . 00 .
2.
Article 11 of the CONTRACT is amended to provide for an
increase in the fee to he paid to Engineer for all work and services
performed under the Contract, as amended, so that the total fee paid
by the City for all work and services shall be an amount of
$162,867.00.
3.
All other provisions of the Contract, which are not
expressly amended herein, shall remain in full force and offeet.

Proposed Amendment No. 1 to City Secretary Contract No. 43953
Dear Mr. Chaubey:
This letter is our formai request for Supplemental Authorization to be compensated for additional
engineering and survey services requested by the City of Fort Worth.
Justification
As a result of the recommendations in the Conceptual Design Report, the City has approved
extending the limits of the proposed segments a total of 2,070 linear feet. This amendment will
provide the funds necessary to perform the survey and design for this additional linear footage.
Scope of Work
The Scope of Work proposed for completion under this contract amendment includes survey and
design for the following changes in segment limits:
o M00304R: removed 400 linear feet of sanitary sewer
o M00367: added 334 linear feet of sanitary sewer
® L5757: added 335 linear feet of sanitary sewer and 958 linear feet of water
o L5430: added 843 linear feet of sanitary sewer
Based upon this Scope of Work, the additional fee proposed for Amendment No. 1 is $31,230.00.
Bidding, Construction, and Post -Construction Phase services will be completed using funds in the
original contract.
