AMENDMENT NO. 1
TO
CITY OF FORT WORTH CONTRACT 53575
This Amendment is entered into by and between the City of Fort Worth (hereafter
"City"), a home-rule municipality, with its principal place of business at 200 Texas Street, Fort
Worth,Texas, and Lorraine Brock, a sole proprietor("Performer"). City and Performer shall be
referred to individually as a Party and collectively as the Parties.
WHEREAS, on February 26, 2020, the Parties have previously entered into City
Secretary Contract 53575 (the"Agreement"); and
WHEREAS, the Agreement was executed for Performer to provide the City with
educational programs focused on home organization and women's entrepreneurship
("Performance"); and
WHEREAS, the Agreement began on February 15, 2020 ("Effective Date") and is set to
expire on February 14, 2021 ("Expiration Date")with annual renewal options; and
WHEREAS, the Agreement provided that additional Performance times and locations
may be added by written mutual agreement of the Parties and any such additional Performance
shall be subject to the terms and conditions of the Agreement as though it was included at the time
of execution; and
WHEREAS, the Parties wish to amend the Agreement to reschedule postponed
Performances and to allow for future Performances to occur online.
NOW, THEREFORE, the Parties, acting herein by and through their duly authorized
representatives, enter into the following agreement:
1.
AMENDMENTS TO AGREEMENT
A. Section II.PERFORMER'S OBLIGATIONS. is hereby amended and replaced in its
entirety with the following:
1. Performer agrees to arrive at the Performance site no later than thirty minutes (30)
prior to start time on the Performance date. In the event that Performer will not be able to
arrive by ten(10)minutes prior to start time on the Performance date,the Performer agrees
to contact Jana Hill,Adult Services Manager, at 817-739-1681 as soon as the Performer is
aware of such delay. If the Performer arrives after the stated program time on the
Performance date and the Performance cannot take place, as determined by City in its sole
discretion, the Performer acknowledges and agrees that Performer will not be paid for the
Performance.
2. The Performer agrees to follow the presentation summary, attached hereto as
Attachment A and incorporated into this Agreement by reference as if fully set forth herein.
The performer shall perform on the dates and times and at the locations as set out in
Attachment B, which is attached hereto and incorporated for all purposes, and at all other

times and locations agreed to in writing by the Parties. For the purposes of scheduling
performances, email confirmations of date and times of Performances shall constitute
written agreement by the Parties.
3. The Performer agrees that some of the Performances will be an online event only,
as specified in Attachment B, using City's licensed webinar solution.
4. The Performer shall provide materials as specified in Attachment A for the
Performance and shall remove all such supplies at the conclusion of the term of each
Performance.
5. The Performer shall sign up as an official City vendor via the City of Fort Worth
website no later than March 7, 2020.
6. The Performer shall provide all personnel necessary to facilitate the safe and
enjoyable participation by the public at the Performance.
B. Section III. CITY'S OBLIGATIONS. is hereby amended and replaced in its entirety
with the following:
1. City agrees to designate a representative to coordinate all services to be performed
pursuant to this Agreement.
2. The City shall provide a clean, climate controlled, well-lighted venue for the in-
person Performance events.
3. City will provide the necessary access and licenses to host any online Performances.
City will provide log-in information to the Performer prior to the Performance date.
C. Attachment B - Performance Dates and Locations. is hereby amended and replaced
in its entirety, as attached to this amendment.

ALL OTHER TERMS SHALL REMAIN THE SAME
All other provisions of the Agreement which are not expressly amended herein shall
remain in full force and effect.
3.
ELECTRONIC SIGNATURE
This Amendment may be executed in multiple counterparts, each of which shall be
an original and all of which shall constitute one and the same instrument.A facsimile copy
or computer image, such as a PDF or tiff image, or a signature, shall be treated as and shall
have the same effect as anoriginal.
